A Resolution of the Miami City Commission accepting the recommendation of the City Manager,
approving the findings of the Evaluation Committee, pursuant to Request for Proposals ("RFP") No.
243229, with Mount Sinai Medical Center, for the provision of Annual Physical Examination Services
for First Responders, for the Department of Fire -Rescue and Police, authorizing the City Manager to
negotiate and execute a Professional Services Agreement ("PSA"), in substantially the attached form,
for a period of two (2) years, with the option to renew for two (2) additional two (2) year periods,
allocating funds from the various sources of funds of the user departments, subject to availability of
funds and budgetary approval.

Subject: RRFP 243229 Annual Physical Examination Services for First Responders
Background Information (Continued):
The City of Miami Department of Fire -Rescue and Police are in the need of securing Annual Physical
Examination Services for First Responders ("Services"). These Services include the provision of annual
physical examinations, including Hepatitis A and B immunizations, for Fire -Rescue personnel and
sworn Police personnel. To fulfill this requirement, RFP No. 243229 was issued to secure a provider to
perform these Services for the City of Miami Department of Fire -Rescue and Police.
Based on an evaluation of the Proposals by the Evaluation Committee, and approved by the City
Manager, it is recommended that the City Commission accept the findings of the Committee and
authorize the City Manager to negotiate and execute a Professional Services Agreement ("PSA"), in
substantially the attached form, with Mount Sinai Medical Center, the highest ranked firm, for the
provision of Annual Physical Examination Services for First Responders, for two (2) years, with the
option to extend for two (2) additional two (2) year periods. Should negotiations fail with the highest
ranked firm, the City Manager will be authorized to negotiate and execute a PSA with the second -
highest ranked firm, Mercy Medical Development d/b/a Mercy Outpatient Center for Physical
Examination Services. Should negotiations fail with the second highest ranked firm, the City Manager
will be authorized to reject all proposals and reissue an RFP solicitation. Total estimated average annual
amount is $175,000 for Fire -Rescue and $285,000 for Police.
