Crapp, Jr. City Manager Kenneth Robertson, Director Purchasing Department DATE : March 22, 2011 FILE : SUBJECT : Sole Source for The Repair of Q-9 Fire -Rescue Truck REFERENCES: ENCLOSURES: An investigation was conducted by staff to determine whether Ten-8 Fire Equipment, Inc., a Non- Local/Non-Minority vendor, located at 2904 59th Avenue, Drive East, Bradenton, Fl, 34203, is the sole source provider for the repair of Quint 9 ("Q9"). Q9 was involved in an accident resulting in severe damage to the entire passenger side rear box, ladder assembly and other associated damage. Subsequently, this part of the truck has been damaged beyond repair and this section needs to be replaced completely by the manufacturer. This equipment is very complex and custom-made to manufacturer specifications. The manufacturer is Pierce Manufacturing, Inc., and we must send it to their authorized vendor in the State of Florida, Ten-8 Fire Equipment, Inc., because the manufacturer will only provide original. parts to Ten-8 Fire Equipment, Inc. This allows us to stay within manufacturer. specifications. Most importantly, it will meet all NFPA and Safety Standards when the repairs are completed. Q9 is a multipurpose Fire -Rescue vehicle that can be used as both a Ladder Truck and a Pumper There is a need to return this unit to service as soon as possible. This Fire -Rescue truck is assigned to Fire Station No. 9, located at 69 NE 62nd Street. Its dual role is unique. It is a vital piece of our front line operations in response to ensure the safety and well being of the citizens and visitors of the City of Miami. Accordingly, I am recommending that the requirements for competitive bidding be waived, and these findings be approved: Ten-8 Fire Equipment, Inc., a Non-Local/Non-Minority vendor, located at 2904 59`h Avenue Drive East, Bradenton, FL, 34203, is the sole source provider for the repair of Q9 Fire -Rescue truck for the Department of Fire -Rescue. Funding, in an amount not to exceed $79,089.91, is available from the Department of Fire -Rescue Account No. P-40-B72809, T-05, A-1105, E- Equipment, 0-181000 in the amount of $24,84.09 and P-40-B72809, T-05, A-1017, E-Equipment, 0-181000 in the amount of $54,282.82,1for a total contract amount $79,089.91. Kemp, Director Department of Fire -Rescue DATE: SUBJECT: REFERENCES: ENCLOSURES: Request for Sole Source Finding: Authorizing the Repair of Q9 Fire -Rescue Department FILE : This memorandum serves to request that you approve a finding that Ten-8 Fire Equipment, Inc., located at 2904 59th Avenue Drive East, Bradenton, FL, 34203, a Non-Local/Non-Minority vendor, is the sole source provider for the repair of Quint 9 ("Q9"). The Department of Fire -Rescue has received one (1) written quote from Ten-8 Fire Equipment, Inc., in an amount not to exceed $79,089.91, to repair said damages. It is therefore requested at this time, that the Chief Procurement Officer deem this acquisition as a sole source finding and recommend the acquisition of this repair. Funds for this purchase will be allocated from the Department of Fire -Rescue Account No. P-40-B72809, T-05, A-1105, E- Equipment, 0-181000 in the amount of $24,804.09 and P-40-B72809, T-05, A-1017, E-Equipment, 0-181000 in the amount of $54,282.82 for a total contract amount of $79,089.91.
