Gerald J, Lee, Sr. ha been trained in the Graphic Arts Industry now for over twenty years. In 2000, my wife joined me to form a better way of doing business with her background in Public Relations (Ten Years at Ptolemy Group, INC. in Miami's Design District). We were in Dvortown for twelve years 1997-2009 nn NW 12°1 Street and NW ra Ava as Gerald's Graphics J Printing, In 20118 the market turned and we had to continue in a smaller way. With the help of the SEDPW/CRA we can capitalize on a changing industry. We would be able to provide our customers with State of the Art work at a competitive rate. We intend to continue our relationships with some of Miami's mnst prominent companies and organizations, We've done work for GOB, Big -Time Productions, Touching Miami with Love, YWCA, Medical Earn Transportation and the SEDPW/CRA just to show same of the work that we've done. Documentation evidencing source of revenue and financing commitments. 8. Audited financial statements for the most recent reporting period and current year operating budget, Exhibit (B) Project Proposal and Financials 1. Cover letter summarizing the funding request and community benefits. 2, Resumes of officers and key management personnel. 3. Detailed grant proposal that conforms to the objectives of the Southeast Overtown/Park West Redevelopment Plan. 4. Minimum of three detailed quotations for all products and service to be procured through the grant. 5. Detailed project budget reflecting committed and projected sources of revenue and expenses for the proposed project or program. (Include copies of all financial and related contractual agreements, terms and letters of commitment). 6. Two (2) Years of Projected Business Financial Statements (Balance Sheet, P & L, Cash Flow) showing that the business can continue to operate in the next two years after this assistance. 7.
