(RDC) hereby submits this change order revision for phase It gymnasium, concession and classroom addition. The revision includes the addition of the classrooms and the changes to the structure from CMU structure to a CMU/Metal building structure. The scope of work is in accordance with the approved floor plans dated January 31st 2013 as submitted by C3TS and exterior elevations dated 3/21/13. ,Des ian Fees • Provide architectural, structural, mechanical, electrical and plumbing design drawings and construction documents for permitting and construction of the proposed facility. • Provide civil engineering plans and construction documents for the site work development, permitting and construction of the proposed facility. • Provide landscape, irrigation plans and construction documents for the permitting and construction of the proposed facility. ■ RDC to provide Fire Protection Construction Drawings and Mechanical Calculations as per Fire requirements agreed with the City of Miami Fire Department. • Note: Design Drawings and Construction Documents Submittals as follows: Schematic Design, 50% CDs, 90% CD's and Permitting Submittal. Specifications noted on plans. Design as per current code in effect (FBC2010) and Former City of Miami Zoning Code (11000). General Conditions ■ The general conditions cost includes all preconstruction management, project management, supervision, testing, construction survey and stakeouts, temporary site water and electric, temporary storage facilities, temporary toilets, temporary fencing, gates and dumpsters. Bond, Insurances ■ Provide a Payment and Performance Bond, once permits are in place. ■ Provide a Builders Risk Insurance Policy, once permits are in place. Gibson Park Phase II: Gymnasium, Concession, Classrooms & Restrooms Building Page 1 of 6 CG-CA21702 CP01456757 LC13001820 Site Work • The area within the limits of the scope of work will be graded/shaped as required to construct the building pad elevations for the construction of the gymnasium building and parking areas. ■ The existing drainage structure CB 16 and connecting piping shall be removed and relocated and drainage swells shall be re configured to allow for the new building footprint. ■ Accessible concrete sidewalks shall be constructed to connect the gymnasium, classroom entrance and lobby to the west parking lot. • ADA compliant concrete sidewalks will be constructed to provide ADA accessibility to the new proposed gymnasium building and to provide connectivity to the recreation building, multipurpose ball field and site appurtenances. • The concrete sidewalk will be five feet and unreinforced. • A four -inch thick concrete slab shall be constructed on the east side of the concession area to allow for an extended concrete covered outdoor seating area. Site Utilities & Building Services • Existing site.drainage will be adjusted to accommodate construction of the building pad. ��" The; existing sanit�ary'sewer la eral will'beTapped- collect sewage disposal wdef from the new gymnasium, classroom, and restroom building. • A new grease trap shall be construed to collect the water from the kitchen concession ■ The exiting 2" waterdistribution system will be tapped to provide potable water service to the proposed gymnasium / classrooms /restroom building. ■ The existing 8" fire line will be tapped into to provide water required for Fire Protection Services to the new gymnasium, classrooms and restroom building. ■ The existing on site electrical service will be tapped into to provide power required for the new gymnasium building and additional parking lot lighting. Asphalt Parking Lot • Twenty- nine (29)-new asphalt parking spaces will be constructed. • Compliant signs will be provided for each ADA parking space. • The parking light poles shall and light fixtures to match Phase I parking lot. ■ The parking lot shall be lighted with light pole mounted to a height of mounted 20 feet above grade. ■ The light pole / fixture shall be an Oceanic prestresed as manufactured by Sky Cast Inc with a Kim lighting fixture to match the site lighting previously installed in phase I Landscaping and irrigation • All shrubs, ground covers or other planting material installed will be native plantings and conform to the standards of Florida #1. • Landscaped areas will be planted to meet minimum City of Miami 11000 ordinance code requirements. ■ The existing irrigation system will be modified and adjusted to provide coverage to shrubs and ground cover around the proposed gymnasium building and new landscaped parking areas utilizing low angle sprinkler heads, moisture and rain sensors and zoned in accordance to water demands. Gymnasium Area Gibson Park Phase II: Gymnasium, Concession, Classrooms & Restrooms Building Page 2 of 6 • The new gymnasium / concession and classroom spaces shall have a total ground floor area of "Sixteen Thousand and Eight S.F (16,008 SF)" square foot and shall consist of a entrance lobby gymnasium area, men's restroom, women's restroom, cooking kitchen concession, equipment storage area, mechanical, electrical janitor rooms, four (4) classrooms , recreational room, kiln room and computer classroom . • The building foundation and floor shall be monolithic concrete foundation with a 4" reinforced concrete slab on grade. ■ The building structure shall be a pre-engineered steel structure support with metal truss roof and a 24 gauge standing seam metal roof system with 12 feet high 8" reinforced CBS masonry at the lower base of the building. • The upper section of the gymnasium shall be a pre-engineered metal building structure as per attached specification dated 3-18-13. • Roof insulation shall be R 40 gym vanguard vinyl faced insulation • The main east entrance doors at the lobby area shall be impact resistant glass doors. ■ Hollow metal emergency exit doors with panic hardware shall be installed to provide access to the west the building. ■ . Exterior impact fixed glass window systems shall be provided on the, south, east and west elevation of the building, as shown on plans. All glazing frames to be powder coated aluminum framing and meet with Miami -Dade County Product Approval. ■The lower exterior walls will be 5/8" painted stucco finish. • The upper exterior walls finish will be a pre-engineered metal building structure as per attached specification dated 3-18-13. • The interior masonry walls of the building will be finished with 5/8" impact resistant drywall boards and shall be painted. ■ The gymnasium ceiling will be exposed metal joists and shall be painted. Acoustical banners will be attached to the metal joists structure. • The main basketball court playing surface area will be 84'-0" x 50'-O" to meet the National High School Federation Standards. The marking for the two practice courts shall be 42'-0" x 50'-0". • The gymnasium wood flooring surface shall be provide by the City of Miami and installed by RDC • The gymnasium wood flooring will be a grade two and better Hard Wood Maple Flooring with 25/32 x 2-1/4" standard strip laid over two (2) layers of 518" plywood with a vented base cove. The floor will be striped for the main court (84'-0" x 50'-0"), two (2) half -courts (42'-0" x 50'-0") and a volleyball court. • Note: Material for the Wood Floor surfaces shall be provide by the owner and installed by RDC ■ Six (6) adjustable ceiling -suspended rear -folding basketball backstops with glass backboards and breakaway goals as manufactured by Draper Inc., will be supplied and installed. The basketball goals supplied will be two (2) for the main basketball court (50'-0" x 84'-0"), and four (4) for the (42'-0" x 50'-0") parallel courts. All basketball backstops shall be equipped with electric motorized vertical height adjusters. • One electrical motorized roll up curtain divider as manufactured by Draper Inc shall be installed to separate the two practice courts. ■ A 2" thick x 6 foot high safety wall padding panels, as manufactured by Promats Athletic will be provided behind the basketball goals at both ends of the main court area. • The gymnasium will be designed to have a seating capacity of 260 spectators. ■ One set of four rows & one set of three rows telescopic manually retractable aluminum seating bleachers as manufactured by Interkal Seating shall be provided and Installed (Total Seating Capacity (260 Spectators). ■ The gymnasium floor will include provisions for multiple uses including the installation of two (2) brass sleeves for volleyball standards. Gibson Park Phase ll; Gymnasium, Concession, Classrooms & Restrooms Building Page 3 of 6 • Two (2) basketball scoreboards model #BB-1600-4 as manufactured by Fairplay or Equal shall be supplied and installed. The scoreboards shall have wireless controllers and receivers and shall be battery / electrically operated. Scoreboards will be installed to operate independently for the main courts and the smaller courts • A city of Miami logo shall be provided at the center of the main court. The logo shall be 12 feet in diameter. • Acoustical panels and/or banners shall be provided in the ceilings and walls area of the gym as required by code for sound level decibels. Per NRC to equal .5 minimum. • Two (2) ADA compliant, fully recessed, electric drinking fountains will be installed in the gymnasium area and one (1) one the gymnasium exterior. • A cooking kitchen and concession shall be built out for the concession area. The kitchen equipment shall be as per the attached Kitchen equipment List dated 1/31/13 • The. concession kitchen walls shall be finished with and FRP panel • Kitchen and Concession area flooring shall be quarry tile. • Metal Halide Light Fixtures (or equal) will be installed to uniformly light the gymnasium playing and spectator areas at 30-foot candles average as required by the current IES standards. • The security and monitoring system for the building shall be tied and connected to the existing park security system ■ A fire alarm service will-5e designed andirTifalledinlhe complete building: ■ Three (3) fire extinguishers will be provided at the gymnasium area to meet NFPA and Florida Building Code requirements. • An automatic wet -pipe Fire Sprinkler System will be installed in the complete building in accordance with the current code ■ The entire building mechanical HVAC System will be a split Unit System designed with high efficiency packaged type air-cooled chiller. A metal cage will be provided to enclose the unit. • A lightning protection system in accordance with NFPA 780 and UL Publication 96A shall be designed and installed. Classroom, Computer Rooms and Restroom Area. ■ All classrooms, restrooms and exit doors shall be hollow metals doors in hollow metal frames. • RDC to provide a master keying system for the gymnasium, classroom and restroom buildings. • All exterior glass and glazing shall be impact resistant. • Interior glass, glazing and vision lights shall be non -impact %" tempered glass shall be provided in classroom doors. • The partition walls in the restroom area shall be finished with 5/8" durorock board over 3- 5/8" studs partitions = The partition walls in the classroom and computer area shall be finished with 5/8" gypsum drywall board over 3-5/13" studs partitions • A five feet ceramic tile wainscot shall be provided over the restroom interior walls. • The ceilings in the classroom area shall be an acoustical ceiling laid in a metal ceiling grid ■ The ceiling in the men's and women's restroom shall be finished with a 5/8" gypsum boards and shall be painted = All classrooms and corridor flooring areas shall receive a VCT floor with a 6" VCT base • Flooring in the restroom and janitor shall receive an epoxy floor finish. • Four (4) moveable partitions shall be provide and installed. The moveable partition shall be a Sound Transmission Coefficient "STC" of 42 ■ Laminated plywood counter tops shall be provide and installed in the computer rooms. Gibson Park Phase II: Gymnasium, Concession, Classrooms & Restrooms Building Page 4 of 6 • The men's restroom will be equipped with one (1) ADA water closets, one (1) regular water closet, two (2) ADA lavatories, one (1) regular lavatory and two (2) urinals. Fixtures and finishes to match existing Recreation Building as per City Of Miami request. • The women's restroom will be equipped with one (1) ADA water closets, three (3) regular water closet, two (2) ADA lavatories and one regular (1) lavatory. Fixtures and finishes to match existing Recreation Building as per City Of Miami request. • The unisex restroom shall be equipped with One (1) ADA water closet and one (1) ADA lavatory. • Three (3) ADA compliant, fully recessed, electric drinking fountains will be installed on the exterior of the classroom area. • Toilet.partitions and accessories and manufactured by Bobrick or equal shall be installed in both men's and women's restroom. Partitions and Toilet Accessories to match existing Recreation Building as per City Of Miami request. • Light fixture in classrooms will be fluorescent lay in light fixtures, ■ The security system for the classrooms and computer rooms shall be tied and connected to the existing park security system • Data and telephone wiring shall be provided for the classrooms building and computer rooms • Electrical and data requirements shall be installed for the connection of the Smart Board System. Smart Board Equipment shall be supplied by Owner (NIC). General Notes & Provisions • RDC will provide civil, landscape, irrigation, architectural, structural, mechanical, plumbing and electrical plans necessary to permit and construct within the scope of work. • RDC will be responsible for obtaining local and state permits. (All fees are reimbursable). ■ RDC will be responsible for the revision of the DERM drainage improvement permit. (All fees are reimbursable). ■ RDC shall be responsible for obtaining Miami Dade Water & Sewer approval for revisions to the existing for Water & Sewer Improvements. (All fees are reimbursable), • RDC requests that as this is an addition to the project, the retainage for the previous contract will be released after the close documents have been delivered to and approved by CIP and before the final completion of the new gymnasium building. • Warranties for the previous contract will not be extended to the completion of the gymnasium building. • Phase 2, Class 11 permit revision drawings will be provided by RDC as per meeting with Zoning Department. • Kiln and/or other specialty equipment not noted in this scope of work shall be provided by Owner. • Parking Count Code compliance to be waived by the Zoning Department. Responsibility of the City of Miami.
