Sample Parent Contract
Brickell Preparatory Academy
As a parent of Brickell Preparatory Academy, I understand the importance of the parent contract as
my expressed commitment to assist the school in creating a safe, positive and academically enriching
school environment. I understand and agree to comply with the following policies and procedures
which have been explained to me at the time of registration:
1. Attendance- Parents must notify the office if a student is going to be absent. On the day the
student returns to school, he or she must bring a note from the parent explaining the reason
a for the absence. Students are allowed five (5) excused absences per semester.
ci73 2. Tardies - Parents are to ensure that their child arrives to school on time.
—I a. Students must be in their seat by their assigned arrival time.

b. Any student arriving after this time will be issued a late pass.
—1" c. After ten (10) unexcused tardies per school year, mandatory parent conference will
r) be held as the student may be issued a referral form to be placed in the student's
0 permanent record.
3 Parental Involvement - We believe that parents play an integral role in their child's
1 -1- educational and social life. For this reason, parents are required to volunteer at least 30
Di hours per year to the school.
a. All hours must be completed prior to May 31st of the school year. In order to be
eligible to reregister for the following academic year, 50% of the annual volunteer
r) hour commitment must be completed by February.
b. The School will provide various options to complete these hours.

(13 4. Transportation - Parents are required to make transportation arrangements for their child
and complete a transportation form at the time of registration notifying the school of their
transportation plan. The following are acceptable modes of transportation:
a. Walking/Biking/ Public Transportation - Students walking, biking, or taking
public transportation to and from school must abide by all school policies and public
safety rules. Students biking to school MUST wear helmets and lock bicycles in
designated bike areas.

c. Failure to complete the 30 hours will affect the child's registration for the following
year.

b. Private Vehicles - To ensure the safety of all students, parents driving to school will
receive a route map and vehicle decal upon registration and MUST abide by the
designated ingress and egress routes set by the school.
i. Non-residents of "The Roads" are PROHIBITED from using the following
streets when transporting students to and from the school: 19th , 20th ,
22nd, 23rd or 24th Road from Coral Way to SW IAvenue.
ii. Decals must be properly displayed on the front windshield driver's (visible
to school staff) in order to expedite drop off and pick up.
iii. Car Pools - Carpooling is highly encouraged, as it positively contributes to
children's social development and encourages community engagement.
Parents who sign up for carpooling through the school will receive up to 5
credit hours per week towards their volunteer commitment. Parents with car
pool decals may NOT park in carpool lane and walk their child to the
classroom.
c. Failure to comply with any and all transportation rules set by the school will result in
disciplinary action and will affect the child's registration for the school following
year.
5. Uniforms - School uniforms must be worn every day.
a. Parents are to ensure that their child is wearing the proper uniform.
b. Students who arrive to school without the proper uniform will be sent home.
c. White long sleeve shirts may be worn under the school shirt on cool days.
6. Homework - Parents are responsible to sign Friday folders/planners and to check daily and
weekend homework assignments. Students are responsible to complete book reports
monthly.
7. Parent Conferences - Parents must attend all meetings and conferences called by school
administration or educational staff.
8. Emergency Contact - Parents will maintain current emergency contact numbers and up-to-
date telephone numbers,
9. Medication - In the event that a student is on medication, the office must be notified and
parents must complete Authorization for Medication. Medication must be prescribed by a
physician and in a prescription container containing the name of the patient, dosage
amounts, and medication name.
10. Unauthorized Items - Students are not allowed to bring any games, toys, or electronic
devices from home into the classroom. A teacher may confiscate any unauthorized items
that my child may bring into the school. The teacher or administration may keep such items
until the end of the school year.
11. Code of Conduct - As a parent I understand the importance of the School's Student Code
of Excellence and have explained it to my child as detailed in thc Student and Parent
Handbook. Both my child and I agree to adhere to the Code of Excellence and policies and
reu1ations of the Student & Parent Handbook.
I have read the School's Parent and Student Handbook, including the Parent Contract,
uniform policy and Student Code of Excellence and agree to abide by all of the policies and
regulations stated therein.
