Subject: Resolution to accept the recommendation of the City Manager approving the findings of the Evaluation
Committee pursuant to RFP No. 273288 that the highest ranked firm. Humana. Inc., be awarded to provide Employee
Benefit Dental Plan. for both the Dental HMO and Dental PPO Plans for the Department of Risk Management.
• Purpose of Item:
To respectfully request that the City Commission approve the attached. resolution accepting the recommendation of the
City Manager approving the findings of the Evaluation Committee pursuant to RFP No. 273288, and authorizing the
City Manager, after consultation with the City Attorney, to negotiate and execute a Professional Services Agreement
with Humana, Inc., as the highest ranked firm, for both the Dental HMO and Dental PPO plans, in substantially the
attached form, for a period of three (3) years with the options to renew for two (2) additional one (1) year periods,
allocating funds from the City's -Risk Management Budget Account Code 05002.30] 001.523000.0000.00000, subject
to availability of funds and budgetary approval.

Background Information:
On October 6, 2011, RFP No. 273288 for the procurement of an Employee Benefit Dental Plans was issued by the
Purchasing Department at the request of the Risk Management Department. An Evaluation Committee approved by
the City Manager ranked Humana, Inc. as the highest ranked provider of Dental HMO and Dental PPO plans and
recommended that the City negotiate a contract with Humana, Inc. -for both the Dental HMO and Dental PPO plans.
The City Manager approved the findings of the Evaluation, Committee— Should negotiations fail with the highest
ranked firm. Humana, Inc., the City Manager will be authorized to negotiate and execute a PSA with the second -
highest ranked firms, Solstice Benefits, Inc. for the DHMO Plan and Delta Insurance Co. for the PPO Plan, for a period
of three (3) years, with the option to renew for two (2) additional one (1) year periods, with funds allocated from the
City's Risk Management Department, Budget Account Code 05002.301001.523000.0000.00000, subject to the
availability of funds and budgetary .approval at the time of need. Should negotiations fail with the second -highest
ranked firms, the City Manager will be authorized to negotiate and execute a PSA with the third -highest ranked firms,
Delta Dental Insurance Co. for the DHMO Plan and Solstice Benefits, Inc. for the PPO Plan, a period of three (3) years,
with the option to renew for two (2) additional one (1) year periods, with funds allocated from the City's Risk
Management Department, Budget Account Code 05002.301001.523000.0000.00000, subject to the availability of funds
and budgetary approval at the time of need. Should negotiations fail with the third -highest ranked firms, the City
Manager is authorized to reject all proposals and issue a new RFP Solicitation.
