Subject: Request for Proposals (RFP) No. 220240 Physical Examination Services
Purpose of Item:
A Resolution of the Miami City Commission accepting the recommendation of the City Manager
approving the findings of the Evaluation Committee, pursuant to Request for Proposals No. 220240,
with Mount Sinai Medical Center, for the provision of Physical Examination Services, for the
Department of Employee Relations, authorizing the City Manager to negotiate and execute a
Professional Services Agreement, in substantially the attached form, for a period of two (2) years,
with the option to renew for two (2) additional two (2) year periods, allocating funds from the various
sources of funds of the user departments, subject to availability of funds and budgetary approval.

BACKGROUND RE: RFP #220240
The City of Miami Department of Employee Relations D epartment is in the need of securing pre-
employment, promotional, and return to work physical examination services ("Services"). These
Services include the provision of general physical examinations for employees or prospective employees
referred and authorized by the Department of Employee Relations. To fulfill this requirement, RFP No.
220240 was issued to secure a provider to perform Physical Examination Services for the City of Miami
Department of. Employee Relations.
Based on an evaluation of the Proposals by the Evaluation Committee, and approved by the City
Manager, it is recommended that the City Commission accept the findings of the Committee and
authorize the City Manager to negotiate and execute a Professional Services Agreement ("PSA"), in
substantially the attached form, with Mount Sinai Medical Center, for the provision of Physical
Examination Services, for two (2) years, with the option to extend for two (2) additional two (2) year
periods. Should negotiations fail with Mount Sinai Medical Center, the City Manager is authorized to
negotiate and execute a PSA with the second -highest ranked firm, Mercy Medical Development d/b/a
Mercy Outpatient Center for Physical Examination Services. Should negotiations fail with Mercy Medical
Development d/b/a Mercy Outpatient Center, the City Manager is authorized to negotiate and execute a
PSA with the third -highest ranked firm, Metro -Dade Firefighters Wellness Center for Physical
Examination Services. Should negotiations fail, the City Manager is authorized to reject all proposals and
reissue an RFP solicitation. Estimated annual amount: $51,000.00.
