HomeMy WebLinkAboutSpreadsheetDMS Division of State Purchasing has entered into a State Term Contract for Telephony
Equipment and Services (STC No. 730-000-09-1) with Avaya Inc., Cisco Systems, Inc., and
Siemens Communications, Inc. for a three (3) year term beginning on September 3, 2008 and
continuing until September 2, 2011.
This STC is MANDATORY for use by State Agencies and available for use by "Other Eligible
User" as outlined in 60A-1.005, F.A.C. and Section 163.3164, F.S.
PURPOSE:
The purpose of this State Term Contract is two fold:
The first is to facilitate the PURCHASE, or LEASE / RENTAL, of hybrid or "pure" IP Telephone
Systems in the following listed configurations:

The second is to provide a migratory path to certain IP Telephony functionality, as well as
ongoing maintenance (including moves / adds / changes), for the embedded base of "legacy"
telephone systems currently installed throughout the State.
This contract does NOT allow for procurement of any new equipment incapable of supporting IP
functionality.
INSTRUCTIONS for PURCHASE:
Three quotes are NOT REQUIRED for this STC. However, customers are encouraged to
competitively set the price for each particular order by initiating a request for quote ("RFQ") from
among the approved vendors listed on this Contract.
NOTE: The vendors listed above are the ONLY VENDORS AUTHORIZED TO PROVIDE
EQUIPMENT OR SERVICES ON THIS STC.
State Agencies should utilize the State's MyFloridaMarketPlace (MFMP) e-Procurement
system when completing Purchase Orders or Direct Orders.
Certain Florida Administrative Rules will apply when connecting the new system to the State's
Network.*
* Rules 6OFF-1, 6OFF-2, 6OFF-3 encompass SUNCOM customer eligibility, purchasing services, exemption
approvals, and billing, in addition to SUNCOM service usage requirements, network security policies, and the
procedures for maintaining state government listings.
INSTRUCTIONS for LEASE / RENTAL:
In addition to purchase, all equipment and services offered by the vendors on this STC is also
available for Per Licensed Station Lease or Rental by utilizing a SUNCOM Communications
Service Authorization (CSA) form (COM Form 9001). Communications and Information
Technology Services (CITS) will facilitate the lease and rental of new systems. All CSA forms
must be approved by CITS.
The Communication Services Authorization form (CSA) constitutes the complete agreement
among the agency/the State (DMS/CITS) & the Vendor. In the event some type of additional
agreement is signed by any eligible user, it will be considered void. Under no circumstance
shall any vendor directly rent, lease, or manage any voice system component under the
awarded contract without an authorizing CSA form issued to the vendor by CITS. Any changes
affecting billing such as additions, deletions, and modifications to the customer's account and/or
system facilities or equipment will be made solely on information contained on a properly
executed CSA.
Rules 6OFF-1, 6OFF-2, 6OFF-3 encompass SUNCOM customer eligibility, services, exemption
approvals, and billing, in addition to SUNCOM service usage requirements, network security
policies, and the procedures for maintaining state government listings will apply for all lease and
rental services.
EXCLUDED ITEMS:
This State Term Contract is intended to serve the telephony and VoIP needs of the State.
Certain items (networking, peripheral equipment, etc) are available on other STCs and are
excluded from this CONTRACT.
Those items include (but are not limited to) the following:
• PCs, Laptops, Monitors
• Servers
• Routers
• LAN/WAN Infrastructure Equipment
• Storage Devices
• Printers and Scanners
• Video Conferencing and Video Bridging Equipment
• Firewalls / VPNs
• Office technology (copiers, radios, facsimile machines, calculators, telephones,
paper shredders, typewriters, scanners, multifunctional devices etc.)
• IT application and database software
• IT professional and consulting services
This STC is between the State and the vendors listed above. There are NO RESELLERS on this
Contract. However, Customers are encouraged to use any authorized Sales Agents or other FL
Certified Minority Business Enterprises who have been approved to by each Vendor to perform
certain services in accordance with this Contract. Information on Certified Minority Business
Enterprises (CMBE) is available from the Office of Supplier Diversity.
Florida Governor Charlie Crist signed Executive Order 07-126, titled "Leadership by Example:
Immediate Actions to Reduce Greenhouse Gas Emissions from Florida State Government." As
such, certain products offered under this Contract are ENERGY STAR 4.0 and/or EPEAT
Bronze certified, or greater. Customers should use environmentally friendly products whenever
possible.
