Amendment of the Agreement — KF 121

Enclosed, please find for your review, the Extension and Amendment Agreement
between Miami -Dade County, through the Miami -Dade County Homeless Trust
and The City of Miami, Inc. for the above referenced grant. Please carefully
review the amendment of the agreement as well as the attachment. Following
your review, please complete the signature page and return to the Homeless Trust
all three originals of the Agreement, attention Terrell Thomas Ellis, Contract
Monitoring and Management Supervisor.
