HomeMy WebLinkAboutExhibit 2ATTACHMENT A SCOPE OF SERVICES MIAMI HOMELESS ASSISTANCE PROGRAM (MHAP) OUTREACH TO HOMELESS DETAINESS Contract # KF111 SCOPE OF SERVICES The Provider agrees to provide outreach, assessment, referral and transportation services to detainees at the Dade County Jail. The Provider will offer placement in programs to homeless arrestees being released. If placement is refused, information will be provided regarding the Countywide Continuum of Care system of services and provide transportation back to where they were arrested. The project will operate seven days a week, twenty-four hours a day. The project will target only those who are released from jail. The Salvation Army will have set aside six (6) beds for individuals accepting shelter. The provider will coordinate with the Dade County Jail and Pre -Trial Release officials for hours of release and anticipated court times that arrestees may be freed. Each month, for a twelve-month period of this project the provider will: * Contact a minimum of 800 individuals being released from Dade County Jail, resulting in 2400 outreach contacts over the grant term; * Provide transportation to 200 individuals (25%) back to their place of arrest; * Assess the needs and provide shelter referrals to a minimum of 400 homeless persons (50%) * Place a minimum of 200 persons (50%) in housing for more than seven days. Conditions: 1. Reimbursement shall be made only for costs associated with the provision of services actually provided to clients under this Agreement, consistent with the approved budget, Attachment B, unless the County agrees, in writing, to another mode of payment or for an advance as provided for in this Agreement; 2. Monthly reports, should be signed by the Executive Director of the agency and submitted to the County, as required; 3. Detainees in the Dade County Jail are priorities for this program; 4. Outreach teams, consisting of two (2) individuals will be stationed at the Jail to provide outreach, assessment, referral, and services under this Agreement; 5. The Provider will work closely and cooperatively with Miami -Dade County's Department of Corrections and other outreach offices contracted with the County, through the Homeless Trust; 6. The Provider will work closely and cooperatively with the intake staff of the Homeless Assistance Centers and other emergency housing providers in the community. 7. The Provider shall make referrals to other services that are available through the Continuum of Care, i.e., health services, mental health services, non -treatment beds, employment and job training/placement services, housing placement assistance, in accordance with the policies and procedures for said referrals; 8. The Provider shall follow the Outreach and Assessment policies and procedures established by the Miami -Dade County Homeless Trust, to include referrals to the appropriate level of housing and services; 9. The Provider shall place homeless individuals and families in appropriate housing, emergency facilities, transitional, and permanent housing throughout Miami -Dade County;
