The following changes, additions, clarifications, and deletions amend the above -captioned
Bid/Contract documents, and shall become an integral part of the bid responses and the
subsequent contract. Please note the contents herein and affix same to the documents you have
on hand.
Extension of Bid Date:
The original bid date, May 5, 2008 at 2:00 PM has been changed. Blds are due
Monday. May 12.2008 at 11:00 AM.
The deadline for submitting questions (RFI's) remains the same, ADril 25, 2008.
All questions submitted on or before that date will be addressed in Addendum No. 4 which will be
posted on the CIP website.
THIS ADDENDUM IS AN ESSENTIAL PORTION OF THE CONTRACT DOCUMENT
AND SHALL BE MADE A PART THEREOF.

Attached to this Addendum for clarification purposes are additional sketches for Section A7.05. as
follows: 7/A7.05, 8/A7.05 and 9 /A7.05. Please refer to question # 17.
Requests for Information
Question No. 1 Can you please specify the # of ply system for the flat docks and if it
is hot asphalt applied?
Answer No. 1 Refer to the Bid Specifications 3-ply.
Question No. 2 What areas on the flat roof portion are non air conditioned spaces?
Answer No. 2 Room 107, Room 116, Room 132 and Elevator Hoistway are not air-
conditioned. Insulation at these areas is required for roof sloping/drainage
purposes as indicated on the roof plan. All other flat roofs require minimum
R-19 insulation.

Substitute expired NOA indicated in the specifications with NOA: 07-

The Architect is unaware of "Scaloflex° product. Products may be
substituted following the procedures outlined in the Bid documents.
Is there a better detail of the ornamental piece that is required to be
applied to the gutters of the metal roof?
Refer to details 1/A8.01 ornamental gutter.
Question No. 6 TC-7 on S2.02 are shown, but not labeled, shall we include them as a
rising roof level?
Answer No. 6 All tie columns shown in sheet S2.02 shall extend to the roof. The
majority of tie columns are labeled on the Foundation/Slab-on-
Grade plan. Please refer to S2.02 plan note # 5 and column schedule
on sheet S5.02.
Question --Now- ---Section 3 on A5.01 shows double masonry wall on SW exterior
wall not shown on structural, please clarify?
Answer No. 7 Section 3 A5.01 is cutting through TC-4 tie columns, which are 16" wide.
There are no double walls for the project.
Question No. 8 Under the Section 7, Bid Submission, it notes that prices for both
projects need to be included. Does the signalization number need to be
brokenoutfrom the lump sum bid? And if so, does the project number
for the signalization improvements need to be referenced? If so, the
cover sheet of the signalization plan labels the project no as: B-64052,
please confirm.
Answer No. 8 Only one price should be submitted, with a break down for the signalization
indicated in the schedule of values. Delete project # B-64062 as noted on
the cover sheet and replace with correct project number is B-60452.
Question No. 9 There Is no place on the bld documents to define what our company
would be self performing. It is necessary to provide that information
with the bid package?
Answer No. 9 Question No. 8 of the -Questionnaire on page 91 requires the bidder to
provide a breakdown of the subcontractors and the % of work they are to
perform.
Question No. 10 What are the dimensions for the column corner guards at the
Apparatus Room?
Answer No. 11 Column comer guards at Apparatus Room 116 to be 3"x3"x3'-4" high
embedded into concrete; mounted 12" AFF; galvanized and painted.
Question No. 12 Will the entire Hose Hangar System require galvanized metal and
Painted?

Architectural drawings do not indicate a BBQ Grill; however, plumbing
sheet P-3.01 indicates a BBQ grill near the LP gas tank. Will you require
this as part of the General Contractor's scope of work? If so, what is
the model number or specification?

Question 18. WB-1 is only shown on sheet A5.01 and indicates it w111 tre used as
trim piece above the rubber tile base. The general notes for the ID
"drawings -indicate WB-1 as all wall base. ID finish schedule does not
indicate where else this WB-1 will be used. Please clarify:
Answer 18. WB-1 occurs only in Exercise Room 128 above sports tile base. All other
watt base to be vinyl type VB-1.
Question 19. Under the minimum requirements, can a Certified Building
Contractors license from the State of Florida, be substituted for a
Certified General Contractor's license in order to bid on the project
as the primary contractor?
Answer 19. Yes, but you must meet all other minimum requirements as stated in the
bid/contract documents.

Hat furring is typical for non -insulated condition.
1-1/2" "Z-furring" strips are to be used. Note that R-5 rigid board is required
at all the following exterior walls:

Please clarify wall type P-1D. Wall legend indicates "same as P-1
except no furring or gyp. bd on both sides" Locations of this wall
type on the plan view appear to show finishes on the CMU on one or
both sides. Please clarify the locations and construction of this wall
type.
"P-1 D" locations as shown on the drawings are correct. Construction
of wall type "P-1 D" is to be modified to include gypsum board on metal
furring on both sides of CMU.
There are no Geotechnical or Environmental Reports included with
the bid package, where can we obtain copies?
A copy of the Geotechnical report is included as part of this
Addendum. The City of Miami is currently having the Environmental
Assessment Report performed.
Are there As -Built drawings for the existing structures?
There are no As -Built Drawings available at this time.
Who will be responsible for the abatement?
A determination will be made after the report has been received.
What part of this project is "By Owner"?
Fitness equipment in Exercise Room 128, loose furniture (tables and
chairs) thru-out, televisionsree-standing wardrobes and cabinets in
-Ready -Rooms are provid d-by Owner. All other items, equipment,
fixtures, etc., shown on the drawings and/or called for I the Specifications,
as well as times clarified during the Bidding process via Addendum, shall
be furnished and installed by the Contractor.
In Section 16400-14 of the Bid Specifications, "2.04.A.", the specs call
for a minimum of 250kW Generator. On the plans "E5.01 it calls for
180 kW. Which one is required?
The Generator is 180kW.
In Section 16400-21 of the Bid Specifications, "2.09.A", the specs call
for a 1200 AMP Main Une Breaker. On the plans "E5.01", it calls for an
800 AMP Main Line Breaker, which one is required?

Vault, they informed us that they do not build a UL-142 fuel tank but a
UL-2085, please clarify.
Generator does not need an enclosure; it is mounted inside a room.
The same Section for the Fuel Tank also calls for a 1000 gallon fuel
tank and a 3000 fuel tank, please clarify.
The tank is a 3,000 gallon ConVault located outside the building.
The Invitation to Bid clearly stated that 30% of this project must be
self performed. At the Pre -Bid meeting the question was asked
whether General Conditions could be included as part of this
percentage and the answer was no, the 30% must be manual labor.
Can you please confirm this?
The contractor must physically perform 30% of the work.
