A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
ACCEPTING THE RECOMMENDATION OF THE CITY MANAGER APPROVING
THE FINDINGS OF THE EVALUATION COMMITTEE, PURSUANT TO REQUEST
FOR PROPOSAL 50031,THAT THE TOP -RANKED FIRM TO PROVIDE GROUP
HEALTH BENEFITS FOR PART-TIME AND TEMPORARY PERSONNEL IS CIGNA;
AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE A
PROFESSIONAL SERVICE AGREEMENT, IN SUBSTANTIALLY THE ATTACHED
FORM, WITH CIGNA, FOR AN INITIAL PERIOD OF EIGHTEEN (18) MONTHS,
WITH THE OPTION TO RENEW FOR THREE (3) ADDITIONAL ONE (1) YEAR
PERIODS, AT AN ESTIMATED TOTAL CONTRACT AMOUNT OF $720,000;
ALLOCATING FUNDS FROM THE DEPARTMENT OF RISK MANAGEMENT,
ACCOUNT CODE NO. 05002.302001.545000.
WHEREAS, the City of Miami ("City") issued Request for Proposal ("RFP") 50031 for the provision
of Group Health Benefits for Part -Time and Temporary Personnel, for the Department of Risk
Management; and
WHEREAS, three (3) proposals were received, deemed responsive and responsible, and
evaluated by an Evaluation Committee; and
WHEREAS, the Evaluation Committee, appointed by the City Manager, deemed Cigna as the top -
ranked firm and proposal most advantageous to the City; and
WHEREAS, the Evaluation Committee recommended to the City Manager to negotiate a Service
Agreement ("Agreement") with Cigna, for a period of eighteen (18) months, with the option to renew
for three (3) additional one (1) year periods;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI,
FLORIDA:
Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by
reference and incorporated as if fully set forth in this Section.
Section 2. The recommendation of the City Manager approving the findings of the Evaluation
Committee, pursuant to RFP 50031, deeming Cigna as the top -ranked firm, for the provision of Group
Health Benefits for Part-time and Temporary personnel, is accepted.
Section 3. The City Manager is authorized {1 } to negotiate and execute an Agreement, in
substantially the attached form, with Cigna, for an initial period of eighteen (18) months, with the option
to renew for three (3) additional one (1) year periods, for an estimated total contract amount of
$720,000, with funds allocated from the Department of Risk Management, Account Code No.

Section 4. This Resolution shall become effective immediately upon its adoption and signature
of the Mayor.{2}

Footnotes:
{1} The herein authorization is further subject to compliance with all requirements that may be
imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter
and Code provisions.
{2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days
from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.
