Claims
Starbridge Choices clean claims
are processed within 10 business
days of receipt. It is your
employee's responsibility to submit
the requested information in its
entirety before any claim can be
paid. An employee can either
confirm with the Starbridge
Choices Claims Department that
the provider has submitted an
itemized bill or the employee can
submit the itemized receipts along
with a claim form. Only original bills
will be accepted. Employees must
submit the bills by mail as Starbridge
Choices will not accept faxed
copies. Employees should always
make copies of all claim information
for their personal records.
Pre -Existing Conditions
If your employee was previously
covered, he or she may ask for a
Certificate of Creditable Coverage
from the previous insurance carrier
and submit it to Starbridge Choices
along with the first claim form. The
Certificate may reduce or eliminate
the Pre -Existing Condition
Limitation that is mentioned in the
Enrollment Brochure. Submitting
the Certificate with the first claim
form will also speed up claim
processing. If your employee did
not have prior group insurance, he
or she may be subject to the Pre -
Existing Condition Limitation
Clause. Any questions regarding
pre-existing conditions can be
referred to our Customer Service
Department at 1-877-209-7098.

insurance card so their doctor can
verify benefits.
Paying for Services
Starbridge Choices is easy to use
because most providers bill us
directly. However, some providers
may ask your employee to pay for
services before leaving the physician's
office. In this case, your employee
should pay for the services, then file a
claim with our Claims Department.
About Starbridge Choices, A CIGNA Health Insurance Plan
Starbridge Choices provides medical coverage for your employees by offering
help with basic medical expenses. Starbridge Choices can help with common
problems like the flu or a broken bone which can become stressful events for
your employees and lead to absenteeism and low morale.

Enrollment Periods
There are certain opportunities in
which your employees can enroll in
Starbridge Choices. Once these
opportunities have ended, your
employees may have to wait until
the annual open enrollment period
before they can enroll. Special
circumstances outside of Open
Enrollment or New Hire Enrollment
may allow an employee to enroll or
make changes as a "Special
Enrollee." Please contact our
Customer Service Department at

employee qualifies.

through Friday 5:OOam-6:00pm MST.
