HomeMy WebLinkAboutExhibit2-SUBAgency name: Attachment A SCOPE OF SERVICES City of Miami Police Department Program name: Violence Intervention Project (VIP) of Little Haiti, North Miami, North Miami Beach Contract Amount: $26,764.80 Program Intent: This program will use community based solutions to reduce violence and to stop child and youth murder in the Area 4: North Miami, North Miami Beach, Little Haiti area(s) in which violence and other negative societal indicators have correlated throughout this year and in historical periods of violence. This will be accomplished by the organization of a group of agencies and individual partners from a variety of disciplines and backgrounds to develop and support this initiative. 2. Target Population: 300 3. Service Delivery (Activities & Outputs): Activities/Service Name & Description Minimum # of Participants to be served Outputs GREAT Training -Juvenile Investigations Unit will implement GREAT at elementary schools in Little Haiti. 300 youths. 12 GREAT training. Provider shall report output data in Data Tracker through the data collection clerk on a basis as outlined in Attachment D of this contract. As applicable, Provider shall report in narrative form, the reasons for non -acceptance into the program, dropping out and failures to achieve outputs, as well as describing any factors that affected provision of services. 4. Performance Measures: The Provider shall adhere to the following performance based outcome measures: Measurement Too, 80% of youth demonstrate increase in knowledge about gangs and violence. All contracted staff working in the program must comply with background screening and fingerprinting requirements, in accordance with Florida Statues Chapter 435 and Miami - Dade County Background screening requirements, as applicable. Providers must maintain staff personnel files which reflect that a screening result of "no findings" was received.
