CCDH is pleased to support United Cerebral Palsy Association of Miami's application to
continue providing Supported Employment services for Miami -Dade County Public School
students who have disabilities and are seeking special diplomas. . Services Contract.
. United Cerebral Palsy Association of Miami is a pioneer in the provision of supported
employment services, having begun to do so in the early 1980's. It has a proven track
record of providing services to individuals whose significant support needs typically
preclude their employment. The agency is known for its professionalism and expertise in
this, and all areas of support to individuals with disabilities. UCP and its staff are
dedicated to providing each person every possible opportunity to fulfill their goals and
realize their dreams.
The Miami -Dade County Public Schools would do well for its students by continuing its

The Center on Aging and Disabilities is pleased to support UCP's application to provide
Supported Employment Services for the Education of Students with Disabilities. There is
a state and nationwide initiative underway to offer a greater menu of options to
children and adults with disabilities. Decreasing the number of adults who utilize an
Adult Day progrom in the future depends on providing education and altemative
options as early as possible. Many students, parents, families and guardians may never
consider the transition from school to work as a possibility without support services in
place. These students should have the same opportunity to be included in the
community and competitive employment is certainly one way to ensure that happens.
As Director of the Florida LINCS Project (a national demonstration to increase inclusive
Community service opportunities for persons with disabilities), I am aware that UCP has
been involved in the LINCS program for the past 18 months. Your attendance at our
quarterly LINCS meetings is o strategy you ore using to find ways of helping students
and adults build their menu of experiences, to include service and volunteering. When
they are ready to work, these individuals will then be able to demonstrate a wider
variety of skills.
I am further aware that over the years UCP has shown o long-standing commitment to
providing innovative services to people with disabilities, and has done so with integrity.

It is our pleasure to recommend Ms. Carter and The United Cerebral Palsy supportive
employment program. We have had the pleasure of working with them for the past five
years and hope to continue as long as possible.
Ms. Carter has worked meticulously as a liaison between community businesses and our
students in order to acquire and maintain successful employment for them. Many of
these students have been offered extraordinary job opportunities due to her dedication.
Ms. Carter goes beyond her call of duty by making home visits, assisting parents, training
and monitoring students.

UCP and Mrs. Carter, has helped me with my son Joseph,
thru the years to remain employed. My son can be a
difficult person at times, but they continued to work with
him. He has been employed now for the past 3 years and
they continued to work with him even after he graduated
from high school in 2006. UCP and Mrs. Carter has been a

As a parent of ESE student, I would like to say that
UCP and Mrs. Carter, has been a support to me and my
granddaughter since the time my granddaughter has been in
the supported employment program thru her school.
This year Mrs. Carter plans to have my granddaughter
working in one of the Schools cafeteria which will be a
great help for us. Mrs. Carter goes out of her way to help
assist us in anyway that she can and I'll like to say Thanks.

The Livingston Group Home, Inc would like to take the time to say thank you, for the
wonderful Summer Program that you provided for the children_ Parris and Shawntae
enjoyed going on different types of Field trips and meeting other people. Parris would
like to attend your school because he feel that he will be able achieve his goal, everyone
