The module must be removed and completely rebuilt. However, Medtec is also determined to re -use as many parts as possible. The cost for the new module is $103, 905. In order to properly prepare the chassis for the new module, the following must occur. $ 11,250.00 Removal of module and chassis evaluation $ 1,200.00 Frame Reconditioning $ 250.00 Alignment $ 3,000.00 Delivery to Goshen and Back to Bradenton $ 1,900.00 Final Inspection for (2) Miami Personnel in Goshen, IN $121,505.00 Total Repair Cost Medtec Ambulance Corporation will apply a new vehicle conversion warranty from the date of returned in-service. This includes a 12 month/12, 000 mile base warranty, lifetime module structural warranty, 10 year prorated paint warranty, and a lifetime electrical warranty. These are the warranty terms that we provide to all new rescues. Additionally, Medtec Ambulance recommends, designs and builds ambulances according to the Federal Specifications for Ambulances, KKK-A-1822E, to insure the safety, functionality and reliability of the emergency vehicle. Repairing this unit under this strict standard will provide insurances that this unit will function as it is intended and deliver services to the City of Miami for many years to come. Medtec Ambulance Corporation By M ales Representative Wednesday, November 14, 2007 City of Miami Fire Rescue Tom Klimoski 1151 NW 7th Street Miami, FL 33136 Description Repair Rescue #3, Unit 7075 reposal ➢ Remove the modular box and evaluate the chassis. ➢ Repair twist in frame if necessary. ➢ Manufacturer a new modular box and mount it on the current chassis. ➢ Utilize as many parts as possible off the old box, credits Will be given for each part used. ➢ Final Inspection for (2) from the City of Miami in Goshen, IN ➢ Unit will be driven from Bradenton to Goshen, and back to Bradenton, then to Miami to final delivery. ➢ See attachment for potential credits — o Group A - Credits included in this quote. o Group B — Credits included in this revised quote o Group C — Small chance to re -used, credit not included. Total Price $121,505.00 Total Price $121,505.00 Said apparatus is to be shipped in accordance with the specifications hereto attached, delays due to strikes, war or international conflict, failures to obtain chassis, materials, or other causes beyond our control not preventing, within about 180 — 210 days after receipt of signed written order at our office at Goshen, Indiana, and receipt of chassis, to be delivered to you at Miami, FL. The specifications herein contained shall form a part of the final contract, and are subject to changes desired by the purchaser, provided such alterations are interlined prior to the acceptance by the company of the order to purchase, and provided such alterations do not materially affect the cost of the construction of the apparatus. The proposal for emergency medical apparatus conforms to all Federal Department of Transportation (DOT) rules and regulations in effect at the time of bid, and all (KKK-A-1822E) Federal Specification for Star of Life Ambulance and (AMD) Ambulance Manufactures Division Guidelines for Automotive Emergency Medical Apparatus as published at the time of bid, except as modified by customer specifications. Any increased cost incurred by first party because of future changes in or said DOT, KKK or AMD standards will be passed along to the customer as an addition to the price set forth above.
