Authorizing the Repair of Rescue 3
for Department of Fire -Rescue
I conducted an investigation to determine whether Medtec Ambulance Corporation/Ten-8 Fire
Equipment, Inc., located at 2429 Lincolnway East, Goshen, IN 46526, is the sole source service
provider(s) for the repair of Fire Rescue 3 Vehicle, for the Department of Fire -Rescue.
On April 25, 2006, Fire -Rescue 3 Vehicle, manufactured by Medtec (herein Medtec) Ambulance
Corporation, was purchased utilizing the Florida Sheriffs Association Contract No. 05-06-0823, for an
approximate total purchase price of $201,933 from Ten-8 Fire Equipment Inc. Said Rescue 3 Vehicle
was involved in an accident and is in need of repair. Since Medtec Ambulance Corporation was the
manufacturer of Fire -Rescue 3 Vehicle and Ten-8 Fire Equipment, Inc. is Medtec's sole authorized
representative/distributor for the state of Florida, this vehicle can only be repaired by Medtec via the
request of the original seller and sole authorized representative/distributor for the state of Florida, Ten-8
Fire Equipment, Inc. Fire -Rescue 3 Vehicle is in need of having the modular box rebuild and the chassis
repair to a new and safe condition. Ten-8 Fire Equipment, Inc. will apply a new vehicle conversion
warranty from the date of return/in-service of Fire —Rescue 3 Vehicle, including a 12 month/12,000 mile
base warranty, lifetime module structural warranty, 10 year prorated paint warranty, and lifetime
electrical warranty.
Accordingly, I am recommending that the requirements for formal competitive sealed bidding be waived,
and these findings be approved: Medtec Ambulance Corporation (the manufacturer) and/or Ten-8 Fire
Equipment, Inc., Medtec's sole authorized representative and distributor for the state of Florida, is the
sole source provider(s) for the repair of Fire Rescue 3 Vehicle, for the Department of Fire -Rescue.
Funding, in an amount not to exceed$121,505, is available from Account Code No. P-18-109001, T-
01.01, A-1017, E-Equipment, 0-184010.
