Re: Amendment to Medical Supplies Contract,

Dear Ms. Butler:
This amendment is issued to include language that was inadvertently left out of Section No. 2.16 of the
Special Conditions of Bid No. 34019,4. Section 2.16 should read as follows (deleted language has been
stricken out and new language is in bold and underlined):
2.16. ADDITIONS/DELETIONS OF FACILITIES/ITEMS/PRODUCTS
Although this solicitation identifies specific facilities/items/products to be—ser;;sed, it is
hereby agreed and understood that any related facility/item/products may be added to,
and any awarded items/products may be deleted from, this
contract at the option of the City. When an addition to the contract is required, the
successful bidder(s) under this contract and other suppliers, as deemed necessary,
shall be invited to submit price quotes for these new cervices/items/products. If these
quotes are comparable with market prices offered for similar sew-ices/items/products,
they the supplier(s) and item(sl shall be added to the contract, w iicheveF if it is in the
best interest of the City and an addendum and a separate purchase order or change
order shall be issued by the City.
