Subject: A Revocable License Agreement (RLA) between the City of Miami ("City") and Concept
House, Inc., a Florida non-profit corporation, to allow for the use of city -owned property located at 4850
NE 2"d Avenue, Miami, Florida.
Purpose of Item:
A Resolution of the City of Miami Commission, authorizing the City Manager to execute a Revocable
License Agreement ("Agreement"), in substantially the attached form, between the City of Miami and
the Concept House, Inc., a non-profit corporation ("Licensee"), for the use of city -owned property
located at 4850 NE 2nd Avenue, Miami, Florida by the Licensee, on a month -to -month basis for the
purpose of providing substance abuse treatment and outpatient services for at risk youth, including but
not limited to individual and group counseling, family therapy, life skills training, school drop -out
prevention and educational sessions such as drug refusal skills, problem solving, relapse prevention for at
risk youth, as well as related administrative offices; and providing for the payment of an annual Use
Fee ("Use Fee") of One Hundred Dollars ($100.00) and a guarantee deposit of $500.00 ("Guarantee
Deposit"); further authorizing the City Manager to execute all necessary documents for said purpose,
with terms and conditions as more particularly set forth in said Agreement.

Subject: A Revocable License Agreement (RLA) between the City of Miami ("City") and Concept
House, Inc. a Florida non-profit corporation, to allow for the use of city -owned property located at 4850
NE 2'"i'Avenue Miami Florida.
Background Information:
Concept House, Inc., a 501[c] 3 non-profit corporation ("Licensee"), has been operating a substance abuse
treatment and outpatient program for at risk youth at this city -owned property under Revocable Permit
Agreement since June 1, 1988. The original Revocable Permit expired in 1998 and provided for
unlimited one year renewals at the discretion of the City Manager. Concept House, Inc. was
incorporated in 1970 and currently provides substance abuse treatment and outpatient services to
approximately forty-eight (48) adolescents, aged 12 to 18 per year at said location. The Administration
has prepared a Revocable License Agreement (RLA) through which this organization may continue to
provide its services at this location.
Under the terms of the RLA, the Licensee will be responsible for maintenance of the Property and all
utilities.
