HomeMy WebLinkAboutExhibit 3THE CITY OF MIAMI, FLORIDA DEPARTMENT OF PUBLIC WORKS APRIL 24, 2007 REPORT OF PROPOSED RECORD PLAT OF HIGHLAND PARK ESTATES LOCATED ALONG N.W. 13 STREET BETWEEN N.W. 9 AVENUE AND N.W. 10 AVENUE The accompanying Plat entitled HIGHLAND PARK ESTATES was prepared by Fortin, Leavy, Skiles, Inc. It is in correct form for submission to the City Commission and is forwarded with a recommendation that it be approved. PERTINENT INFORMATION REGARDING THE PLAT: (1) The property platted is a replat of the property described in the attached Legal Description. The area platted consists of one (1) tract of land containing approximately 50,488 square feet (1.16 of an acre). It is zoned R-4. (2) The location of the streets and their widths conform to the standards of the Department of Public Works of the City of Miami, Florida. (3) As certified to by Daniel C. Fortin, Registered Surveyor and Mapper, this Plat complies with the plat filing laws of the State of Florida. (4) The City Zoning Board of Miami, Florida, after Public Hearing, has recommended the closing of that portion of the 10 foot wide alley of Block 18 of HIGHLAND PARK (Plat Book 2 at Page 13) lying between N.W. 13 Street and the 836 Expressway. This action was confirmed by City Commission Resolution No. R- 06-0193. (5) The Certificate of Title Examination dated 4/)f t/4 ,ZB47 , signed by rit i i .g&•%vim , Attorney, indicates that the fee simple title to the property platted is correctly vested in Jackson Residences, LLC, a Florida limited liability company, and that the plat has been correctly executed. The area platted is encumbered by a mortgage and the mortgage holder has executed the plat and joined in its dedications. In accordance with the requirements of Chapters 54 and 55 of the Code of the City of Miami, Florida, a Performance Bond, in the amount of $120,065.00, has been executed by Jackson Residences, LLC, Principal, and Lincoln General Insurance Company, Surety, for the subdivision improvements at HIGHLAND PARK ESTATES. The Performance Bond will accompany the Agreement -between the City of Miami --and Jackson Residences, LLC, to guarantee the following: 1. REMOVE APPROXIMATELY 3,837 SQUARE FEET OF EXISTING SIDEWALK 2. REMOVE APPROXIMATELY 593 LINEAR FEET OF EXISTING CONCRETE CURB & GUTTER 3. REMOVE APPROXIMATELY 1,818 SQUARE YARDS OF EXISTING ASPHALTIC CONCRETE SURFACE 4. CONSTRUCT APPROXIMATELY 680 LINEAR FEET OF 6-INCH CONCRETE CURB & GUTTER 5. CONSTRUCT APPROXIMATELY 3,231 SQUARE FEET OF NEW 4-INCH CONCRETE SIDEWALK (INCLUDING HANDICAP RAMPS) 6. CONSTRUCT APPROXIMATELY 308 SQUARE FEET OF NEW 6-INCH CONCRETE SIDEWALK 7. CONSTRUCT APPROXIMATELY 2,086 SQUARE YARDS OF NEW ASPHALT PAVEMENT (1 '/2 INCH ASPHALTIC CONCRETE PAVEMENT AND 8- INCH LIMEROCK BASE) 8. PROVIDE AND INSTALL APPROXIMATELY 100 LINEAR FEET OF 24- INCH PERFORATED ALUMINUM FRENCH DRAIN PIPE 9. PROVIDE AND INSTALL THREE (3) TYPE "F-3" CATCH BASIN WITH CUSTOMIZED FRAME AND COVER 10. PROVIDE AND INSTALL TWO (2) TYPE "A" MANHOLES WITH CUSTOMIZED FRAME AND COVER 11. PROVIDE AND INSTALL APPROXIMATELY 87 LINEAR FEET OF 16-INCH REINFORCED CONCRETE PIPE 12. CUT TWO (2) OPENINGS TO EXISTING F3 CATCH BASIN AND CONNECT NEW 15•RCP 13. RELOCATE AND ADJUST TWO (2) EXISTING WATER METER BOXES TO NEW GRADE 14. ADJUST FOUR (4) EXISTING SANITARY SEWER MANHOLES, FRAME & COVER TO NEW GRADE 15. REMOVE AND DISPOSE TWO (2) EXISTING DRAINAGE INLET 16. ABANDON 30 LINEAR FEET OF EXISTING STORM SEWER PIPE (8) The attached Resolution has been prepared for the acceptance of the Plat by City Commission of Miami, Florida.
