HELPING KIPS PREPARE FOR LIFE
Partnership Agreement
Program Summary:
Communities In Schools of Miami, Inc. (CIS) serves as the lead agency for the Violence
Intervention Project (VIP) through the Children's Tru'sl Child Murder and Youth Violence Prevention
Initiative. City of Miami Police Department, hereafter referred to as contractor, will collaborate to
deliver the VIP project.
VIP is a collaborative effort among Communities In Schools of Miami, PanZOu/DJJ, SantLa,
Children's Psychiatric Center, North Miami Police Department, North Miami Beach Police
Department, City of Miami Police, and Village of El Portal Police Department. Each party agrees to
work together to accomplish all the goals, objectives, and activities as prescribed in The Children's
Trust contract and to adhere to the contractual agreements set forth by The Trust. For specific
details of these requirements, please see attached , contract — Attachment F. For your
convenience, below are highlights from the contract, but this is not an exhaustive list of their
requirements and contractor must assume responsibility for familiarity with the Children's Trust
requirements and expectations.
Funding from the Children's Trust funding is renewal based on effective goal attainment and
funding availability.

Data Collection: Contractor agrees to incorporate Children's Trust forms into their enrollment
package. See Attachment B for Children's Trust forms. Data required for Tapestry data entry will
be submitted by the 5th of each month to the lead agency.

Target Population: See Scope of Services - Attachment A.
Amount Payable: CIS agrees to pay the contractor $43,500. See Attachment C for detail budget.
Method of Payment: By the 5th of each month, contractor will submit an invoice to CIS with
accompanying copies of receipts, timesheets, and/or other documentation to support the financial
request. This invoice will be incorporated into CIS monthly reimbursement request to the
Children's Trust. Payment is not rendered to the contractor until CIS receives the monthly
reimbursement from The Children's Trust. Anticipate a 45-60 day period from receipt of your
invoice. It is critical that timely utilization of funding occurs.
Budget: Contractor agrees to provide matching funds and inkind services as needed to complete
reporting requirements to The Children's Trust. Changes of more than 10% in any line item must
be approved by The Children's Trust.
Grant Period: February 1, 2007-January 31, 2008
Data Management: Contractor may be required to submit appropriate back-ups during
programmatic and financial audits conducted by the Children's Trust. The Children's Trust
mandates that individual files be maintained on each student. See Attachment D for required data
reporting to the Children's Trust.
Reporting: See Attachment E for schedule of reports required by The Children's Trust.
Contractor's due date will a week earlier as indicated on the attachment.
Personnel Fi es: he Children's Trust requires that personnel filesora11 employees paid
Children's Trust funds contain at minimum:
1. Resume and job application
2. Job Description signed and dated by employee
3. Sign acknowledgments of personnel policies to include grievance procedures, drug free
environment, adherence to federal Civil Rights and Equal Employment policies, and
acknowledgement of receipt of training on reporting client abuse and client confidentiality.
4. Copies of state and national background screening and moral affidavit.
5. Evaluation/Reviews
6. Training Log
7. Copies of any employees' certifications, degrees, diplomas, etc.
Contractor agrees to make personnel and program participant files available to Children's Trust
audit reviews.
Marketing: Contractor agrees to acknowledge the Children's Trust and incorporate their logo onto
any project related materials including applications, surveys, etc.

The following statement must appear in materials pertaining to any programs funded by the
Children's Trust for this VIP project: (Name of Program) is partially funded by The Children's Trust:
The Trust is a dedicated source of revenue established by voter referendum to improve the lives of
children and families in Miami -Dade County.
Risk: Contractor and CIS agree to continually maintain liability, workman's comp, directors and
officers, auto and non -owned insurance coverage of its board and staff and volunteers throughout
the duration of the VIP program. Minimum limits_.are as follows:
• Comprehensive general liability — no less than $500,000 combined single limit per
occurrence for bodily injury and property damage,
• Automobile liability insurance covering all owned, non -owned, and hired vehicles in an
amount not less than $1,000,000 combined single limit per occurrence for bodily injury and
property damage.
Proof of insurance may be requested from either party by either party at any time.
Contract Renewal: Contract renewal is contingent on funding availability, successful completion
of scope of services, and mutual desire to continue the partnership.
Other conditions: Each agency agrees to retain program and fiscal files for a period of up to five
years. Contractor agrees to serve on the VIP Steering Committee and collaborate with all partners
working in support of VIP.

• This agreement is not intended to create or infer a separate organization, but rather the
collaboration of already -ex s its ii ng non-profit organizations.
• This agreement is limited to the Children's Trust grant in support of the VIP project and is
not related to other efforts of either organization.
• Contractor does not assume responsibility and shall not be held liable for the
conduct/actions of any CIS board, staff, or volunteers, nor any incidents that occur in the
absence of contractor's board, staff, or volunteers.
• CIS does not assume responsibility and shall not be held liable for the conductlactions of
any contractor's board, staff, or volunteers, nor any incidents that occur in the absence of
CIS board, staff, or volunteers.
CIS of Miami & City of Miami Police Department project enter into this agreement effective
February 1, 2007. CIS will give ten (10) days notice in writing to terminate the agreement for
reasons of funding availability as per The Children's Trust contract.
