(PEICO) proposes to provide the services identified below for the project entitled Civic Towers, pursuant to its Professional Service Agreement with the City of Miami for Miscellaneous Consulting Services, dated April 29, 2004. I. GENERAL Pursuant to your request, PEICO understands that the Pollution Rernediation Section of the Department of Environmental Resources Management (DERM) requires a Source Removal Action at the above -referenced facility. Source removal activities will be conducted on the southeastern and northern portions of the above -referenced property in accordance with PEICO's DERM-Approved Source Removal Plan (PEICO Project Number 130818), dated May 15, 2006. li. SCOPE OF WORK Task 1 Site preparation PEICO will contact a local utility locator service at least 72-hours prior to conducting any on -site activities in order to allow local utilities an opportunity to mark and identify potential conflicts in the source removal areas, PEICO will present notification letters to the property owner for their subsequent distribution to the residents detailing the scope of the project and expected duration. Source removal areas will be secured with a temporary 6-foot chain link fence restricting access until project related activities conclude. A security guard will patrol "exclusion zones" to protect and minimize unauthorized access. July 25, 2006 WORK ORDER PROPOSAL PEICO Page 2 Task 2 Soil excavation and disposal PEICO will provide the equipment, labor and materials necessary to excavate previously - identified impacted soil, in accordance with the DERM-Approved Source Removal Plan. PEICO will excavate to a depth of two -feet below the land surface; the excavated soil will be temporarily stockpiled on -site both on and underneath impermeable plastic sheeting to ensure dust control and prevent exposure. Excavated soil will be properly disposed by licensed waste disposal firm. Task 3 Application of Engineering Control PEICO will apply engineering controls in accordance with the DERM-Approved Source Removal Plan. Specifically, in the excavated areas, PEICO will apply geotextile fabric and backfill with two -feet of clean fill material followed by sod for soil stabilization. In areas where excavation to a depth of two -feet is not feasible (i.e., in the vicinity of mature vegetation and the creek bank wall) PEICO will grade 6 Inches below land surface, apply clean fill provide the equipment, labor and material to install concrete pavers to act as an approved engineering control. Task 4 Preparation of Source Removal Report PEICO will prepare a Source Removal Report for the site in accordance with Section 24- 11.1(2)(1)(3) of Chapter 24 of Code of Miaml-Dade County. The report will describe the methodology and results of the source removal, including conclusions and recommendations. III, SUBCONSULTANTS The below listed Sub -Consultants will assist in the performance of the Work. SubConsultant Name none Specialty or Expertise IV. SCHEDULE OF WORK — TIME OF PERFORMANCE Within 7 business days of receipt of authorization to proceed, the outlined scope of services will be initiated, from date of initiation the source removal field activities will be completed within thirty days and a report of findings will be submitted to the City of Miami within twenty days after completion of the source removal field activities. V. COMPENSATION Consultant shall perform the Work detailed in this Proposal for a Lump Sum fee of $90,977.70. Said fee includes an allowance for Reimbursable Expenses required in connection with the Work, which shall be established in an amount not to exceed $100.00. Said Reimbursable Expenses shall be utilized in accordance with the Agreement Provisions and shall conform to the limitations of Florida Statutes § 112.061.
