HOMELESS SERVICES — OPERATIONAL COSTS
1. Services are to be provided at the Lotus House which is located at 217 N.W. 15th Street,
Miami, Florida 33136.
2. SUBRECIPIENT must ensure that residents meet one of the following eligibility criteria and
maintains appropriate documentation according to HUD guidelines:
• Persons living on the street or in short-term emergency shelter,
• Persons coming from transitional housing for homeless persons.
• Persons being evicted from a private dwelling.
• Persons from a short-term stay (up to 30 consecutive days) in an institution who
previously resided on the street or in an emergency shelter.
• Persons being discharged from a longer stay in an institution.
• Persois fleeing domestic violence.
3. SUBRECIPIENT will provide transitional housing and supportive services to at least 30
women. This will be documented by occupancy records and case files
4. SUBRECIPIENT understands that the payments are to be used for the operational costs of
the shelter which may include (but are not limited to) the following:
• Maintenance
• Operations
• Rent
• Repairs
• Security
• Fuel
• Equipment
• Insurance.
• Utilities.
• Food
• Furnishings
• Salaries
The SUBRECIPIENT also understands that not more than 10 percent of the grant amount
may be used for costs of staff (excluding maintenance and security salary costs).
5. SUBRECIPIENT understands and agrees that it must prepare and submit, on a quarterly
basis prior to the 20th day of the following month, the attached Quarterly Report of
Accomplishments.
6, The work performed under this Work Program shall be subject to inspection and approval by
the City.
