The original Memorandum of Understanding entered into between your organization and the Department of Financial Services, Division of State Fire Marshal, expires on August 30, 2006 and must be amended before that date to extend the life of the contract. The contract extension date currently approved by DCA is December 31, 2006. We are seeking an additional modification to extend the contract to February 28, 2007 and do not anticipate that to be a problem. The Department is providing you the opportunity to extend the terms of your existing contract with the Department until, and including December 31, 2006. To accept this offer. your acceptance by close of business Friday, August 25, 2006. With your documented acceptance, the effective date of the modification of the agreement shall be August 29, 2006, and the ending date will December 31, 2006 (and we will be pursuing the February 28, 2006 date). If you do not respond within the timeframe specified, the original contract end date of August 30, 2006 will still be in effect and you will not be able to submit for any further reimbursements for this contract.
