Recommend 10% increase effective 12/15/05, 10% increase effective 12/15/06, EA increase effective 7/15/07. Also, $10,000.00 Incentive effective 12-15-05 and 12-15- 06. (No annual payout} 2 Days/Yr (Each January} (No annual payout) 11 Days/Yr. {F.t time of separation: payment of all accumulated vacation and sick leave) Insurance Health Life Accidental Death .and Dismemberment Insurance Long Term Disability - : Choice of 2 medical plans: An indemnity/network plan through CIGNA Insurance with deductible, and one HMO Plan through CIGNA Insurance. Enrollment is optional but waiver forms must be •completed if declined. In addition, vision benefits with no, deductible and dental benefits with no calendar year, limit.
