between
CITY OF MIAMI and ENVIROWASTE SERVICES GROUP, INC.
This AGREEMENT ("Agreement"), is made and entered into this day of ,
2006, by and between the City of Miami, Florida, a municipal corporation of the State of Florida,
whose principal address is 444 S.W. 2nd Avenue, 10th Floor, Miami, Florida 33130 ( the "City")
and Envirowaste Services Group, Inc., a Florida corporation, whose principal address is 5931
SW 88 Street, Miami, Florida 33156, ( the "Contractor").
WITNESSETH:
WHEREAS, pursuant to Resolution No. , adopted by the Miami City
Commission on , the City Commission accepted the bid of Envirowaste
Services Group, Inc. (the "Contractor"), to provide additional work required for cleaning the
storm sewer infrastructure caused by the last hurricane season and authorized the City Manager
to enter into an agreement with the Contractor, in the sum of $299,610, for a two year period and
is by this reference incorporated herein (the "Contract"); and
WHEREAS, The City requires additional cleaning services to the storm sewer infrastructure
due to the last hurricane season and the Contractor is willing to provide the Additional Services,
under the same terms and conditions set forth in the Contract for an amount not to exceed
$694,600.93.
NOW, THEREFORE, in consideration of the foregoing, and the promises and covenants
contained herein the parties hereby amend the Contract as follows:
1. ARTICLE 1. SCOPE OF WORK: Article 1 of the Contract is amended to include the
Additional Services, consisting of all labor, materials and equipmentfor the work described in
the Bid Proposal attached hereto and made a part of this Agreement.
2. ARTICLE 2. THE CONTRACT SUM: Article 2 of the Contract is amended to increase
the Contract Sum by Six Hundred Ninety Four Thousand Six Hundred Dollars and Ninety Three
Cents ($694,600.93) for a total Contract amount of Nine Hundred Ninety Four Thousand Two
Hundred Ten Dollars and Twenty Nine Cents ($994,210.93).
3. ARTICLE 4. TIME OF COMPLETION: Article 4 of the Contract is amended to
increase the number of days in which Contractor shall complete the work, including the
Additional Services by two hundred (200) working days.

4. ARTICLE 5. ADDITIONAL BOND: Article 5 of the Contract is amended to require
Contract to provide an additional Performance Bond, or to increase the amount of the
Performance Bond, to cover the performance of the Additional Services.
IN WITNESS WHEREOF the parties hereto have executed this Agreement on the day
and date first above written in five (5) counterparts, each of which shall, without proof or
accounting for the other counterparts, be deemed an original contract
