A RESOLUTION OF THE MIAMI CITY COMMISSION, ACCEPTING THE BIDS OF
CSV-USA INC., YAVOY, INC., DIB/A GUAYABERA, ETC., AND WALPATS
INDUSTRIES, INC., THE LOWEST RESPONSIVE AND RESPONSIBLE BIDDERS,
RECEIVED DECEMBER 14, 2005, PURSUANT TO INVITATION FOR BUDS NO.
04-05-102, FOR THE PURCHASE OF T-SHIRTS, GOLF SHIRTS, GUAYABERAS
AND CUBAVERAS WITH EMBROIDERY AND/OR SILK SCREENING CITYWIDE,
TO BE UTILIZED BY VARIOUS USER DEPARTMENTS, ON AN AS -NEEDED,
CONTRACT BASIS, FOR AN INITIAL TWO (2) YEAR PERIOD, WITH THE OPTION
TO RENEW. FOR TWO (2) ADDITIONAL ONE-YEAR PERIODS, AT AN ANNUAL
AMOUNT NOT TO EXCEED $75,000, FOR A TOTAL CONTRACT AMOUNT NOT
TO EXCEED $300,000; ALLOCATING, FUNDS FROM THE VARIOUS
DEPARTMENTS' GENERAL OPERATING BUDGETS, SUBJECT TO BUDGETARY
APPROVAL AT THE TIME OF NEED.
BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA:
Section 1. The bids of CSV-USA Inc., Yavoy, Inc. d/b/a Guayabera, etc. and Walpats Industries,
Inc., the lowest responsive and responsible bidders, received December 14, 2005, pursuant to
Invitation for Bids No. 04-05-102, for T-Shirts, Golf Shirts, Guayaberas and Cubaveras with embroidery
and/or silk screening Citywide, to be utilized by various user departments, on an as -needed contract
basis, for an initial two (2) year period, with the option to renew for two (2) additional one-year periods,
at an annual amount not to exceed $75,000, for a total contract amount not to exceed $300,000, is
accepted, with funds allocated from the various departments' general operating budgets, subject to
budgetary approval at the time of need.
Section 2. This Resolution shallbecome effective immediately upon its adoption and signature of
the Mayor.{1}

Footnotes:
{1} if the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days
from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto. by the City Commission.
