WATERWAYS ASSISTANCE PROGRAM
APPLICATION EVALUATION AND RATING WORKSHEET
The applicant is to complete this worksheet by entering the project title, applicant name
and answers to the applicable questions. **Do not answer with more than three
sentences.**
All applicants must answer a total of ten questions. All applicants will complete
Attachment E-4 of the worksheet, which includes questions 1 through 6.
Each applicant will then complete one and only one sub -Attachment (E-4 A, B, C, D or
E, questions 7-14) according to the applicant's project type in reference to Attachment B.
The applicant should determine their project type (if the project incorporates more than one
project type) by determining which project type is dominant in terms of project cost.
All other sub -attachments that are not applicable to an applicant's Rroject should not
be included in the submitted application.
