HomeMy WebLinkAboutExhibitAMENDMENT NO. 1 TO CONSTRUCTION CONTRACT CORAL WAY BEAUTIFICATION UPLIGHTING, PHASE I PROJECT B-6450-A This Amendment No. 1 to the Contract dated March 3, 2004 (the "Contract") between the City of Miami, a municipal corporation of the State of Florida ("CITY"), and Gancedo Technologies, Inc. ("CONTRACTOR") for the Project entitled "Coral Way Beautification Uplighting Phase I", Project No. B-6450-A, ("PROJECT") is entered into this day of , 2006. RECITALS WHEREAS, the Contract was entered into pursuant to the approval of the City Commission by Resolution No. 04-0035 dated January 22, 2004 in the total amount of $673,031.00 for the Scope of Work included in the original Contract Documents for the PROJECT; and WHEREAS, it has been determined that additional work and costs associated with the additional work is required to complete the project; and WHEREAS, the City and the Contractor have agreed to the scope and associated costs for the additional work; and WHEREAS, the City Commission has adopted Resolution No. on to approve such additional work and the associated costs; NOW THEREFORE, for the considerations hereinafter set forth: The Agreement dated March 3, 2004 is hereby amended as follows: Scope of Work Cost 1. Increase in construction cost due to a difference in the specified conduit elevation after contract bid. This difference was a result of a permit stipulation from FDOT after contract bid. The required conduit excavation was increased from the specifiedA22d $ 4" to306in depth. ,05d 2. Credits to the City for the following items: PVC used in lieu of galvanized sleeves; cadwelds not required; ground rods not required due to use of approved polymer lids in lieu of the specified metal lids. <Deduct $8,290.00> 3. Increase in contract due to concrete repair required at service point. Add $1,716.00 4. Furnish and install 423 tunnel visors at all existing and new up -lights. Includes labor, equipment, materials, profit, overhead, and incidentals. Add $33,209.00 (ROD-4 Co -dub Hurricanes Katrina and Wilma Damage b. Repair eight (8) light fixtures that were knocked over during the hurricanes. Includes labor, equipment, materials, profit, overhead, and incidentals. 6. Furnish and install four (4) new light fixtures damaged during hurricanes. Re -pipe and re- wire as necessary to achieve an acceptable installation. Includes labor, equipment, materials, profit, overhead, and incidentals. 7. Repair and re -plumb two (2) concrete bases that were knocked over during hurricanes. Re - pipe and re -wire as necessary to achieve an acceptable installation. Includes labor, equipment, materials, profit, overhead, and incidentals. 8. Furnish and install one (1) new concrete base that was destroyed during Hurricane Wilma. Re -pipe and re -wire as necessary to achieve an acceptable installation, Includes labor, equipment, materials, profit, overhead, and incidentals.
