HomeMy WebLinkAboutExhibitall kids can CVS ail kids can® Grant Acceptance Agreement The City of Miami (Riverside Park) has been awarded a CVS all kids can TM grant of $80,000 which consists of $68,000 cash plus Boundless Playgrounds technical services valued at $12,000. Acceptance of this grant commits the applicant to meet all the grant requirements including: * Maintain confidentiality of the award until the official media announcement has occurred * Be represented at the media announcement of CVS all kids can I" program * Build a play environment based on Boundless Playgrounds Performance Criteria with your identified playground vendor * Include CVS/pharmacy associates in the development of the project * Grant naming rights to CVS/pharmacy Charitable Trust and agree to include playground signage, to be approved by the Trust, with one of the requirements below: A. Use the CVS all kids can program name in the NAME of the playground. 1) If the actual LOGO cannot be incorporated in the Playground Sign, the program name should still be present with the standard text or predetermined font 2) If the program name is used as standard text (above), the CVS all kids canTM logo must appear somewhere else on the main signage of the playground OR in the surfacing. 3) Logo placement must utilize 15% of the signage area, OR the logo in the surfacing must be 3' x 3'. B. Prominently display at the playground a 24" by 18" placard, incorporating the CVS/pharmacy logo, the mission of program and brief message from CEO of CVS/pharmacy. C. Boundless Playgrounds logo must appear on the entryway signage in a size no smaller than 6 inches high. Guidelines for the use of the name Boundless Playgrounds will be provided separately.. * Submit progress reports to Boundless Playgrounds in a manner and frequency prescribed by Boundless Playgrounds, but at least every 3 months. Periodic financial updates will also be required * Have your vendor submit production plans/construction documents to Boundless Playgrounds for approval prior to the ordering of any play environment equipment * Arrange for your vendor to provide to Boundless Playgrounds a certificate of insurance that names Boundless Playgrounds and CVS/pharmacy as additional insured upon placement of your equipment order * Arrange for Boundless Playgrounds to conduct an Assessment of the play environment after the installation of the equipment but before the installation of the safety surfacing and correct any deficiencies noted in the report * Submit a report to Boundless Playgrounds from a Certified Playground Safety Inspector certified and currently licensed by the National Recreation and Parks Association located in Ashburn, Virginia, that the play environment has passed a safety audit * Submit a final project evaluation/report, including photos, newspaper articles and video. Other documentation may also be requested. Additional status reports must be submitted within 10 business days of request Payout of the grant award will be: 50% to be released upon approval of your plans/construction documents by Boundless Playgrounds; 25% to be released once the assessment has been conducted by Boundless Playgrounds and any deficiencies have been corrected; 25% to be released upon Boundless Playgrounds' approval of a Certified Playground Safety Inspection report. agree, as the authorized representative of the City of Miami, to the terms of the grant.
