Thompson City Clerk MIAMI RECOVERS The following assistance is provided to aid those residents whose homes were located within the City of Miami limits and were renting units in buildings deemed as "condemned buildings," or to homeowners who suffered roof damage due to Hurricane Wilma. Assistance will be extended to qualifying City of Miami residents on a first come, first serve basis as assistance is based on funding availability. Assistance is extended one time per address. ASSISTANCE FOR RENTERS For renters displaced by Hurricane Wilma, both security deposit and relocation assistance will be provided. To be eligible for this assistance, the applicant must: •have left or be leaving a condemned building located within City of Miami limits •have a condemnation letter and a recent utility bill from the last three months (or other acceptable proof of residency within the City of Miami) with an address matching the one on the condemnation letter •have located a new place to live (can be outside of the City of Miami), have a signed lease stating the exact amount of the monthly rent due, and a W-9 form signed by the landlord (the assistance check will be paid directly to the landlord) Security deposit assistance is provided based on the number of bedrooms of the new place of residence. The chart below outlines the maximum amounts of assistance provided. If the security deposit is less than the maximum provided, the lesser amount will be paid out. # of bedrooms of 0 1 2 3 4 new home Maximum provided Relocation assistance is also provided to cover moving expenses, based on the number of rooms of furniture that will be moved from the condemned residence to the new place of residence (can be outside of the City of Miami). The chart below outlines the maximum amounts of relocation assistance provided, based on the number of rooms of furniture being moved (rooms include bedrooms, living room, dining room, kitchen). In all cases, the relocation assistance check will be paid directly to the applicant (renter). #of rooms of i 1 furniture Maximum provided $ 450 $ 600 $ 775 $950 $ 1,075 $ 1,200 $ 1,325 $ 1,450 ASSISTANCE FOR HOMEOWNERS — ROOF REPAIR ONLY For homeowners whose homes suffered roof damage due to Hurricane Wilma, roof replacement assistance will be available. Preference will be given to retirees living on fixed incomes. To be eligible for this assistance, the applicant must: •present proof of ownership of the property •present proof of insurance (if the property is insured) •be current in their property taxes 0 13 Q �? • be the occupant of the property •have less than 120% of median income (see chart below) # of persons in the household 120% income limit $ 45,480 $ 51,960 $58,440 $64,920 $70,080 $75,360 $80,520 $85,680 The following constitute emergency roofing repairs: •cornice defective and in danger of falling •parapet walls leaning or bulging, in danger of falling •defective rain leaders and gutters, causing water to enter premises •roof leaking or open to elements Please note: If the owner has applied for FEMA assistance online, they must fill in their FEMA number on the Miami Recovers application. Homeowner's assistance provides the homeowner with a loan of up to $15,000 and adheres to the guidelines of the City's single family rehabilitation program. As such, a lien will be placed on the property in order for roofing work to commence. If the applicant's home is insured, insurance coverage must be utilized first towards the cost of roofing repairs; subsequently, City assistance will be utilized to cover the deductible. Should FEMA not reimburse the City of Miami for the roof work, the loan will be forgivable after ten years. Note: Once the application has been submitted to Community Development, those applicants whose property is insured will need to later provide a written document from the insurer stating how much the insurer will cover toward roofing work and how much of a deductible the homeowner will need to pay. Exceptions specified in the saving clause may permit an exemption from tax to continue for certain types of income even after the recipient has otherwise become a U.S. resident alien for tax purposes. If you are a U.S. resident alien who is relying on an exception contained in the saving clause of a tax treaty to claim an exemption from U.S. tax on certain types of income, you must attach a statement to Form W-9 that specifies the following five items: 1. The treaty country. Generally, this must be the same treaty under which you claimed exemption from tax as a nonresident alien. 2. The treaty article addressing the income. 3.
