HomeMy WebLinkAboutExhibit 2Attachment A SCOPE OF WORK The scope of work for this Agreement covers three distinct items: 1) Purchase of equipment to be used for as a mobile training unit. 2) Delivery of Emergency Response to Terrorism (ERT) Training. 3) Training outcome assessment and quality improvement processes. Details of the scope, timelines and payment methods for each item are provided below. Section 1. Purchase of Equipment $15,000 To implement the training program throughout the Miami Urban Area, the UMCRME will purchase the following vehicles and equipment. A. Decontamination, personal protective equipment and respiratory equipment. B. Simulation equipment with associated computer hardware and software. The University of Miami CRME will be reimbursed for actual expenditures for the above equipment upon presentation of invoice copies, proof of payment, title and verification of equipment by authorized City of Miami personnel. Reimbursement is through the City of Miami and the State of Florida and may require a minimum of thirty days to complete. Section 2. Course Delivery $240,000 The course curriculum for the Emergency Response to Terrorism (ERT) course was specifically approved for use in the State of Florida by the Office of Domestic Preparedness (ODP) in May 2004. ERT classes are to be delivered to Miami Urban Area first responders as designated by the UASI Urban Area Administrator. The first responder disciplines targeted for delivery are: fire service, emergency medical services, health care, including hospital personnel, and public health, The ERT course will be provided in a two-day, sixteen -hour format. Compensation will be based on tuition of $300 per ERT class per student as described below. By December 31, 2006, (the ending term of the 2005 UASI Grant), ERT classes will be delivered to a minimum of 700 course participants; the total number of course participants will not exceed 800. Total compensation will not exceed $240,000 (based on 800 course participants.) Classes will be held at the University of Miami CRME and/or at locations throughout the defined Miami Urban Area with the approval of the UASI Urban Area Administrator. The University of Miami CRME will provide the following: registration processes (coordination, advertisement and enrollment), salaries for instructors, use of facilities, equipment, lectures, duplication and provision of training manuals, videotapes and other teaching and testing materials, pre -testing and post -testing, learner demographics, performance data, data analysis by experts, reporting of results, and documentation of class participant information. 1 of3 Attachment A SCOPE OF WORK Tuition payments are restricted to employees designated from the Miami Urban Area (City of Miami, Miami -Dade County and its jurisdictions and area hospitals, Broward County and its jurisdictions and area hospitals, Monroe County and its jurisdictions and area hospitals). Designated employees from the Miami Urban Area jurisdictions, as defined above, will be given first priority for ERT classes. Tuition payment/reimbursement is paid only after class completion. The University of Miami CRME will invoice the city monthly and must provide the City with the following enrollment and registration documentation with the invoice: • Instructor name, class title, class dates and locations. • Class roster with name, social security number, employer, rank/position, work phone and e-mail address of participants. • Class sign -in sheet for each class day with original signatures of participants. Tuition will be reimbursed at the rate of $300 per course (two -days, sixteen hours) for each eligible student for up to 800 Miami Urban Area course registrants. The University of Miami CRME must be notified in writing and/or email within three business days of class of any withdrawal of pre -registered learners. Substitution of personnel is acceptable, but notification is required. The UASI grant will reimburse the University of Miami CRME full tuition for registrants who do not show up for class. UMCRME will supply the City of Miami the number of cancellations four days prior to the class when no substitute has been provided so that the City can attempt to fill vacant seats. The UASI Grant will reimburse the University of Miami CRME full tuition for class participants who do not fully complete the course. Those participants may request to attend another scheduled class at no additional charge to complete course requirements. Section 3. Outcomes Assessment and Quality Improvement $95,000 The focus of the outcomes assessment during this contract period is the hospital module, as the assessment for the prehospital provider will be completed. UMCRME is expected to conduct knowledge assessment of participants both before and after taking the ERT course and skills evaluation during the course. The curriculum, equipment, and course methods are to be adjusted to achieve and sustain a high level of skills acquisition. The UMCRME will develop the assessment tools and provide to the UASI Urban Area Administrator for review and approval. Pre -course and post -course evaluation and assessment will occur for each class. Evaluation results are to be provided to the Urban Area Administrator on a bi-monthly basis. In addition, the following activities will occur as part of class delivery, outcomes assessment and quality improvement: • Meetings of evaluation experts to review testing instruments and analyze data obtained. 2 of 3 Attachment A SCOPE OF WORK • Meetings of content experts including EMS and hospital personnel to modify scenarios and skills stations as necessary. • Ongoing train -the -trainer programs. • Pilot study with refinement of testing instruments. • Full implementation with pre- and post -course testing. • Ongoing presentation and evaluation of prehospital curriculum. Payment for outcomes assessment and quality improvement will occur with the approval of the UASI Urban Area Administrator as follows: Development of pre -course and post -course assessment tools (upon approval of tools by UASI Urban Area Administrator). Submittal and approval of quarterlyly assessment and quality improvement reports (to be paid quarterly not to exceed three payments of $20,000 for the 2005 UASI grant). Final report on knowledge and skills advancement in the Urban Area as a result of the ERT course offerings (upon approval of UASI Urban Area Administrator). $ 15,000 $ 60,000 $ 20,000 The following table is provided as a summary of compensation allowable under the Urban Area Security Initiative Grant for 2005. 2005 UASI Grant Grant Ending Term 2/28/07, or until all deliverables are met Section 1.
