Director Chie City Manager PaaP 1 of 1 Agenda Item Summary Form Background Information: The City of Miami issued an RFP for sworn police annual physical examination services which are required pursuant to the FOP labor agreement. On September 23, 2004, pursuant to Resolution No. 04-0617, the City Commission authorized the City Manager to execute an agreement with Mercy Medical Development d/b/a Mercy Outpatient Center. The City entered into an agreement with Mercy Medical Development d/b/a Mercy Outpatient Center effective November 23, 2004 for a two-year period with the option to renew for two additional two-year periods, at an estimated annual amount not to exceed $415,240. Based on a larger quantity of additional/optional testing being conducted, on an as needed basis, funds have been used more rapidly than anticipated, requiring an increase in the annual contract.
