COST/FEE PROPOSAL Cost/Fee Proposal EMS medical direction services will be provided by the Emergency Care Center of Jackson Memorial Hospital in consideration of the following: (Detail outlined in Figure 1.) Optional Associate Medical Director: Not included in the above proposal or in the detailed budget in Figure 1 are costs associated with an Associate Medical Director. TCalculated from the 1997-8 Composite Fringe Benefit Rate (CFBR) of the institution issuing the paycheck to the individual physician. The CFBR is 20% for JMH hires. For the University Of Miami School Of Medicine, the CFBR is 22.5%; the Dean's Office Tax on all professional income is 2.6%. Salary for the Medical Director is paid by the PHT to the University under the Annual Operating Agreement between the institutions. UM then issues a single paycheck to the physician that Includes her compensation for services rendered via the UM Medical Group Practice. 8. MINORITY/WOMEN PARTICIPATION The Public Health Trust of Miami -Dade County / Jackson Health System as a public health resource is compliant with EEOC and Affirmative Action and is proposing a female Medical Director, Kathleen Schrank, MD. 9. LOCAL PREFERENCE Response: The primary office location of the proposer is: The Public Health Trust/Jackson Memorial Hospital is located within the city limits of Miami at 1611 NW 12 Avenue, Miami, Florida 33136-1094. The primary office location of the proposed Medical Director, Dr. Kathleen Schrank, and the location from which she will be based to perform the work is in the Emergency Care Center of the University of Miami/Jackson Memorial Medical Center. The physical address is consistent with the one given above. 10. TRADE SECRETS EXECUTION TO PUBLIC RECORDS DISCLOSURES: Not applicable. There are no trade secrets in the Proposal. 47
