HomeMy WebLinkAboutExhibit 3THE CITY OF MIAMI, FLORIDA DEPARTMENT OF PUBLIC WORKS AUGUST 18, 2005 REPORT OF PROPOSED RECORD PLAT OF HAZELTON PARK FIRST ADDITION LOCATED AT THE NORTHWEST CORNER OF S.W. 8 STREET AND S.W. 52 COURT The accompanying Plat entitled HAZELTON PARK FIRST ADDITION was prepared by Manuel G. Vera and Associates, Inc. It is in correct form for submission to the City Commission and is forwarded with a recommendation that it be approved. PERTINENT INFORMATION REGARDING THE PLAT: (1) The property platted is a replat of the property described in the attached Legal Description. The area platted consists of one (1) lot containing containing approximately 5,672 square feet (0.13± acre). It is zoned C-1. (2) The location of the streets and their widths conform to the standards of the Department of Public Works of the City of Miami, Florida. (3) As certified to by Manuel G. Vera, Registered Surveyor and Mapper, this Plat complies with the plat filing laws of the State of Florida. (4) The City Zoning Board fo Miami, Florida, after Public Hearing, has recommeneded the closing of a portion of right-of-way at the Northwest corner of S.W. 8 Street and S.W. 52 Court (Plat Book 42, Page 52). Thls action was confirmed by City Commission Resolution No. 03-883. (5) The Certificate of Title Examination dated //l-Te.G1 ,Za�� , signed by 63Qrp6 4' Z. , Attorney, indicates that the fee simple title to the property platted Is correctly vested in M.E.D. of Miami, L.L.C., a Florida limited liability company, and that the plat has been correctly executed. (6) The area platted is not encumbered by mortgages. (7) In accordance with the requirements of Chapters 54 and 55 of the Code of the City of Miami, Florida, a Cashier's Check, in the amount of $4,190.00 has been executed posted by M.E.D. of Miami, L.L.C., Owner, for the subdivision improvements at HAZELTON PARK FIRST ADDITION. The Cashier's Check will accompany the Agreement between the City of Miami and M.E.D. of Miami, L.L.C., to guarantee the following: REMOVE APPROXIMATELY 370 SQUARE FEET OF EXISTING CONCRETE SIDEWALK CONSTRUCT APPROXIMATELY 70 LINEAR FEET OF 6-INCH CONCRETE CURSE AND GUTTER CONSTRUCT APPROXIMATELY 265 SQUARE FEET OF 4-INCH CONCRETE SIDEWALK CONSTRUCT APPROXIMATELY 105 SQUARE FEET OF NEW 6-INCH CONCRETE SIDEWALK CONSTRUCT 42 SQUARE YARDS OF NEW PAVEMENT (8" LIMEROCK BASE AND 1" ASPHALTIC CONCRETE SURFACE (8) The attached Resolution has been prepared for the acceptance of the Plat by the City Commission of Miami, Florida. 5;;/"...1— Frank R.
