Dear Mr. Castillo:
Attached is the copy of the Agreement for State Homeland Security Grant Programs
between your county and the Department of Community Affairs for Fiscal Year 2004-2005. We
request that you make three additional copies of the Agreement and that all four be returned to
the Department after you have completed the items listed in this letter.
Please read the Agreement and attachments thoroughly, particularly the items in
bold type. The amount of the Agreement represents an allocation approved for all four
components that are included in Attachment A - Budget and Scope of Work. You will need to
complete the Budget Detail portion of Attachment A (page 20) according to the allocations that
you have accepted from the Department for each component.
Before returning the Agreement, be sure to complete the following:
• All four copies of the Agreement need original authorized signatures
• Authorized person's name and title printed where indicated
• Date of signature
• Federal Identification Number
• If signed by anyone other than the authorized person, a copy of an existing
Certificate, Resolution, or Procedure, or a letter signed by the authorized
person indicating that they have the authority to enter into Agreements.
The Agreement will begin on July 1, 2004 and all allowable costs incurred beginning on
that date may be reimbursed under this award regardless of the final execution date of the
Agreement. The Agreement termination date is September 30, 2005,

If you intend to request an advance, please make sure that you read and complete all
items identified under Paragraph (16) Funding /Consideration. The amount which may be
advanced may not exceed the expected cash needs within the first three (3) months of the
Agreement term. The request must also be submitted on Attachment E with a detailed
justification.
Please note that three of the completely executed contract copies will be retained within
the Department of Community Affairs and one will be returned for your files. If additional
originals are needed, please make the necessary copies prior to signing. The Agreement
should be returned to the attention of Paula Churchwell, Planning Manager, Division of
Emergency Management, 2555 Shumard Oak Boulevard, Tallahassee, Florida 32399-2100. Any
questions concerning the contents of the Agreement may also be directed to Ms. Churchwell at
paula.churchwell@,dca.state.fl.us, or you may contact her at 850-413-9942. If you have any
questions that relate to the Scope of Work and associated work products, please contact the
appropriate person identified in the scope of work for that particular subject area.
