TENTATIVE PLAT #1642-A'
Located between Brickel! Avenue and South Miami Avenue,
Southwesterly of SW 15 Road

The City of Miami Plat and Street Committee, at its meeting of September 22, 2004,
approved the above tentative plat subject to the following revisions being made to the
tentative plat, additional informationbeing provided and/or variances being granted.
Please be advised that the processing of your tentative plat cannot proceed until these
conditions have been satisfied.
1. In the location sketch:
a. Provide a sectional/fractional legal description.
b. Verify zoning designations and zoning district lines.
2. Provide a distance and a bearing for each the monument line and the centerline of
South Miami Avenue and Bickel! Avenue and for each proposed lot line.
3. Elevations must refer to the City of Miami Datum (Add the word "datum').
4. Provide a complete tree survey with species, height, diameter and spread.
5. Is there any structure on the property? If not, label it as vacant.
6. Provide an opinion of title for the remnant portion of Lot 15.
7, A plat approval letter from the F.D.O.T. is required at the time of final plat submittal.
A copy of the final plat must be submitted to the F.D.O.T., not a copy of the tentative
plat.
8. An opinion of title in the City of Miami opinion of title form must be provided at the
time of the final plat submittal.
9. All encroachments across lot lines must be removed prior to the final plat submittal.
10. Tentative plat application must be made with Miami -Dade County after receiving
approval from the City of Miami Plat and Street Committee.
11. Be advised that backup documentation will be required for all who execute the final
plat. A resolution for authority to execute documents and a Certificate of Good
Standing from the Secretary of State is required, If applicable.

12. Before final plat submittal to the City, a copy of the final plat and opinion of title must
be submitted to Miami -Dade County for review and then retrieved and submitted to
the City.
13. In the title block, correct the Plat Book number for "Flagler'", 5 not.J.
14. In the title block, include the name of the City of Miami.
15. Provide a letter from the Zoning Administrator that the location of the Zoning district
boundary line, and the proposed lot line accordingly, are correctly shown on the
tentative plat.
16. All existing structures must be removed prior to final plat.
17. Please see the Miami -Dade water and sewer Department comments enclosed with this
letter.
18. To the zoning information, add the statement, ".... according to City of Miami Zoning
Ordinance 11000, as amended, Article 4, Section 401, Schedule of District Regulations".
in addition to the above requirements, you should be aware of the following:
1. State and local laws require the installation of various physical improvements In the
public rights -of -way when property is platted. These subdivision Improvements
include paving, drainage, landscaping, sidewalks, etc. In some cases this could
represent a substantial investment on your part.
2. The alteration, relocation or installation of utilities such as storm and sanitary sewers,
electric, telephone, water, etc. caused by this plat will be at .the property owner's
expense. Also, utility easements may be required on.the property being platted.
3. A building permit will not be issued on the property being platted until the final plat
• is recorded. Also, the Certificate of occupancy for any building construction .will be
issued only after all the required subdivision Improvements have been completed.
4. Approval for fire flow requirements must be obtained from the Fire -Rescue
Department prior to the Issuance of a building permit.
5. Additional Items must be provided to the Clty of Miami Department of Public Works
before the final plat is submitted to the Clty Commission for approval. You will be
notified in writing as to what these Items are after the amount of the bond has been
determined for the necessary subdivision improvements.
6. Tentative plat approval Is only valid for one i1} year from the date of the Plat and
Street Committee meeting at which time It was approved.
