Code: 55062020628 Vendor No.: F596000376097 nancial Assistance Number: 55004 THIS AGREEMENT, made and entered into this day of by and between the STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION, an agency of the State of Florida, hereinafter referred to as the Department, and CITY OF MIAMI 444 S.W. 2ND AVENUE, 3RD FLOOR, MIAMI FL 33130 hereinafter referred to as Agency. WITNESSETH: WHEREAS, the Department and the Agency heretofore on the 12th day of January , 1999 entered into a Joint Participation Agreement; and WHEREAS, the Agency desires to accomplish certain project items as outlined in the Attachment "A" appended hereto; and WHEREAS, the Department desires to participate in all eligible items for this project as outlined in Attachment "A" for a total Department Share of $1,000,000.00 NOW, THEREFORE THIS INDENTURE WITNESSETH: that for and in consideration of the mutual benefits to flow from each to the other, the parties hereto agree that the above described Joint Participation Agreement is to be amended and supplemented as follows: 1.00 project Description: The project description is amended DESIGN AND CONSTRUCTION OF THE PUBLIC HELIPORT FACILITY AT THE CITY OF MIAMI 2.00 Project Cost: Paragraph 3.00 of said Agreement is decreased by $-7,400,838.00 bringing the revised total cost of the project to $ 2,000,000.00 725-030.07 PUBLIC TRANSPORTATION 07f02 Pape 2 of 5 Paragraph 4.00 of said Agreement is decreased by $-3,700,419.00 bringing the Department's revised total cost of the project to $ 1,000,000.00 3.00 Amended Exhibits: Exhibit(s) B AND C of said Agreement is amended by Attachment "A". 4.00 Contract Time: Paragraph 18.00 of said Agreement NOT APPLICABLE 725-030-07 PUBLIC TRANSPORTATION 07/02 Page 3 al 5 Financial Project No. 25416819401 Contract No, AG101 Agreement Date Except as hereby modified, amended or changed, all other terms of said Agreement dated January 12th, 1999 and any subsequent supplements shall remain in full force and effect. Some of the changes were the elimination of the regional visitor's center and the status of Chalks Seaplane Base being unknown. Therefore, the design was much larger than is needed now. Based on recent conceptual planning, the only component likely to move forward in the near term and remain aviation related is the new City of Miami Public Heliport. The project scope is limited to design and construction of the Watson Island Public Heliport Facility located in the City of Miami.
