Subject: Request to Rescind Award for Bid No. 02-03-161, Forklift Maintenance and Repair to A& S
Equipment Co., as Primary Vendor and Award to Secondary Vendor, National Lift Truck Service
Purpose of Item:
The Purchasing Department is requesting for the City Commission to rescind the award for Bid No.
02-03-161, per Resolution 03-931, Forklift Maintenance and Repair from A& S Equipment Co., of
P.O. Box 228043, Miami, Florida 33122, a Dade / Hispanic firm, as Primary Vendor and award to the
Secondary Vendor, National Lift Truck Service of 1901 NW 2 St, Ft. Lauderdale, Florida 33313, a
Non -Local / Hispanic firm. A & S Equipment Co. has been ordered repeatedly to provide the City
with the required level of corporate and garage keepers insurance in order to operate the contract with
the City for forklift repairs and maintenance - citywide. They have failed to do so.
Background Information:
The initial contract period of this bid expires October 8, 2005 and has two remaining Options -To -
Renew available. National Lift Truck Service, the Secondary Vendor, is in compliance with insurance
and stands ready to continue the agreement. The Purchasing Department sent A&S Equipment Co. a
final Notice of Default on February 4, 2005 with, again, no response. Therefore, it is requested that
the City Commission rescind the award to A&S Equipment Co. and award the Bid to the Secondary
Vendor, National Lift Truck Service of 1901 NW 2 St, Ft. Lauderdale, Florida 33313, a Non -Local /
Hispanic firm.
