HomeMy WebLinkAboutexhibit3aEXHIBIT A STATEMENT OF WORK FOR SERVICE PROVIDERS April , 2005 — June 30, 2005 1. TARGETED JOB SEEKER AND EMPLOYER POPULATIONS A. Residents of the City of Miami are the targeted job -seeker population. B. Construction -related employers with work sites and job opportunities within the City of Miami are the targeted employer population. il. SERViCES Employees of the Service Providers designated in this Agreement shall perform the following services. A. Recruiting residents of the City of Miami to offer employment services; and, recruiting construction -related employers to obtain job openings and offer placement services. B. Screening City residents according 10 employer criteria and referring qualified residents for employment. Screening may include a telephone or personal interview, technical skills assessment, character assessment, and/or drug screening. C. Referring residents to other services provided by the aforesaid Service Providers which may include screening and referral to non -construction job openings for which they qualify, WIA Adult/Dislocated assisted core and training services, WT training services, and self-service job searches. D. Posting employer job openings in the One -Stop Management Information System. E. Monitoring placements of qualified residents for 30 days to assist in the successful retention of the resident. F. Providing residents and construction -related employers with informational and educational literature regarding workforce development services provided by the aforesaid Service Providers. III. PERFORMANCE A. The Service Providers' achievement of the following Objectives and Outcomes shall constitute successful performance of the services funded through this Agreement. Both Service Providers shall be responsible for the recruitment and placement of residents in a sufficient amount to meet the Program Outcomes set forth below. Both Service Providers shall work together to implement the program objectives and achieve the required program outcomes. Program Objectives: ] . To place residents of the City of Miami in jobs with employers whose worksites are located in the City of Miami. 2. To achieve a thirty (30) day job retention for hired residents. Page 7 of 10 Program Outcomes: 1. A minimum of thirty-three (33) residents shall be hired by employers whose worksites are located in the City of Miami. 2. A minimum of twenty-five (25) residents shall achieve thirty (30) day retention. Retention may occur subsequent to Contract expiration date. B. PERFORMANCE BONUS A performance bonus of $300 per thirty (30) day retention shall be paid if the aforesaid employees of the Service Providers designated in this Agreement document the achievement of any thirty (30) day retention in excess of the required twenty-five (25). The bonus shall be split equally among the aforesaid employees. The maximum additional thirty (30) day retention shall not exceed twelve (12). The total maximum compensation under this performance bonus shall not exceed S3,600 in total and $1,200 per employee. IV. EMPLOYMENT AND RETENTION VERIFICATION The aforesaid Service Providers shall document and verify the placements and the thirty (30) day retention after job placement for all residents using the Employment Verification Form (EVF). The original Employment Verification Forms shall be attached to the Monthly Activity Reports submitted by Service Provider to SFWB, Inc. V. REPORTING REQUIREMENTS The aforesaid Service Providers shall submit Monthly Activity Reports to the SFWB, Inc.'s Contracts Manager by the 7`h of the month following the month in which services were rendered. Other reports and information may be required by SFWB, lnc. and shall be prepared and submitted by aforesaid Service Providers in accordance with SFWB, lnc.'s instructions and requirements. VI. FILE MANAGEMENT REQUIREMENTS The aforesaid Service Providers shall maintain a hard copy participant file, which at a minimum shall contain the following: assessment data, proof of eligibility, job referral(s), case notes, follow-up documentation for thirty (30) day retention after job placement. VII. PAYMENT STRUCTURE SFWB, Inc. agrees to pay from the Grant Funds provided by the City all budgeted costs including performance bonuses incurred by the aforesaid Service Providers in accordance with the Budget incorporated herein and attached hereto as Exhibit C. The amount of funding payable to the aforesaid Service Providers under this cost reimbursement/ performance contract shall be: $3 8,791.25 for SER Jobs For Progress, Inc. $24,708.75 for Youth Co -Op, Inc. VIII. METHOD OF PAYMENT A. The aforesaid Service Providers shall provide a monthly itemized invoice package with the required supporting documentation (payroll registers, invoices, receipts, EVFs for thirty (30) day retention performance bonus) to the SEWS, Inc. no later than the seventh (7th) business day of the month following the month in which services were rendered in accordance with this Statement of Work.
