Subject: Supplemental Agreement Request
Dear Mr. Lago:
Metric Engineering, Inc., respectfully requests compensation for the additional work for the NE
71 st Street Drainage Project. The additional work consist of:
Task No. 1: Additional Work to Modify Existing Seawall -
1. Additional survey along the seawall and bay bottom, which is required for the seawall
modifications (such as location of existing piles, pipes, and anchors).
2. Additional structural design work for the changes to the seawall, which is
also required since the as -built plans for the existing seawall were not available.
Task No. 2: Auger Hole Design on NE 701h Street -
1. Percolation testing at NE 701h Street.
2. Design of seven auger hole drainage systems at NE 70 St .
Task No. 3: Milling & Resurfacing --
1. Extending the current project limits and revising the current plans to address the milling
and/or resurfacing for NE 69, 70, 71, and 72 Streets. (Please note that this additional
work extends beyond the present project limits, since currently only NE 72 St drainage
area extends from Biscayne Blvd to Biscayne Bay, with survey performed only where the
drainage structures are to be located.) Limits of milling and resurfacing to be represented
in the plans by means of a line diagram, as no survey will be performed.

2. Details for the milling and/or resurfacing pavement design, ab ,veil as quantities for the
swale improvement, curb and gutter replacement, sidewalk replacement, and ADA
compliance, (which the City is providing) and their accompanying details, (which Metric
will provide), will be drafted into the plans.
3. Pavement marking replacement for the milled and/or resurfaced roads will be handled
with the following note: "Contractor to restore pavement markings to their original
condition.
4. The technical specifications for all the above work (with the exception of the sidewalk
specifications which is already shown in the specifications package) will also be
prepared.
Task No. 4: FDOT Permit & MOT Coordination —
1. Preparation of an FDOT permit for work occurring in or near their right of way is also
required. [This includes milling and resurfacing of NE 69, 70, 71, and 72 Streets as the
work will begin at Biscayne Blvd (SR 5); and for the temporary opening of the existing
road closures at NE 70, 71, and 72 Streets to accommodate maintenance of traffic
during construction.]
Task No. 5: Inventory —
1. Preparation of an inventory of the existing drainage inlets and/or grates, in which
damaged items will be tabulated and shown in the plans as to be replaced.
Task No. 6: Coordination —
1. Includes coordination with City for the above -mentioned issues.
Your consideration of this matter will be greatly appreciated. The engineering, geotechnical and
survey fees for the above work total not to exceed of $20,250. A breakdown of the hours and
fees are enclosed. The timeline for this supplemental work shah follow the original contract
timeline in which work will be completed by August 30, 2004. Should you have any questions or
