DEPARTMENT OF HEALTH, BUREAU OF CHILD NUTRITION PROGRAMS ("STATE") TO
PROVIDE REIMBURSEMENT FOR THE PROVISION OF FOOD TO THE PARTICIPANTS OF THE
CITY OF MIAMI'S 2004-2005 OUT -OF -SCHOOL TIME PROGRAM; AUTHORIZING THE CITY
MANAGER TO EXECUTE AN AGREEMENT, IN SUBSTANTIALLY THE ATTACHED FORM,
WITH THE STATE FOR SAID PROGRAM; AND TO EXECUTE THE NECESSARY DOCUMENTS
TO IMPLEMENT ACCEPTANCE OF THE GRANT AWARDS.
