A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
ESTABLISHING A SPECIAL REVENUE FUND ENTITLED: "AED/STATE OF
FLORIDA EMS MATCHING GRANT M4067 AWARD (FY 2004)," TO SUPPORT
THE PRE-HOSP1TAL ACTIVITIES AND OTHER EMERGENCY MEDICAL
SERVICES ("EMS") ITEMS AND APPROPRIATING FUNDS, IN THE AMOUNT OF
$203,000, CONSISTING OF A GRANT, IN THE AMOUNT OF $152,250, FROM THE
STATE OF FLORIDA DEPARTMENT OF HEALTH, BUREAU OF EMS, FLORIDA
EMS GRANT PROGRAM FOR EMS ORGANIZATIONS, WITH REQUIRED
MATCHING FUNDS FROM THE CITY OF MIAMI ("CITY"), IN THE AMOUNT OF $
50,750; ALLOCATING SAID MATCHING FUNDS FROM THE CITY'S OPERATING
BUDGET, ACCOUNT CODE NO. 001000.921002.6.270; AUTHORIZING THE CITY
MANAGER TO EXECUTE THE NECESSARY DOCUMENTS TO IMPLEMENT
ACCEPTANCE OF THE GRANT.
WHEREAS, the leading cause of death in the United States is sudden cardiac arrest, killing over
400,000 people each year; and
WHEREAS, pursuant to Ordinance No. 12402, adopted September 11, 2003, the City of Miami
("City") initiated a Public Access Defibrillation Program in an effort to increase survivability from sudden
cardiac arrest; and
WHEREAS, the City has been awarded a grant, in the amount of $152,250, for the period
beginning March 31, 2004, from the State of Florida Department of Health, Bureau of Emergency
Medical Services ("EMS"), which, pursuant to §401.113(2)(b), Florida Statutes, provides grants for the
improvement, expansion and continuation of pre -hospital activities and other emergency medical
services items; and
WHEREAS, matching funds from the City, in the amount of $50,750, is required as a condition of
said grant, and is available from the City Operating Budget, Account Code No. 001000.921002.6.270;
and
WHEREAS, as stipulated in the grant award, the City Department of Fire -Rescue will use said
funds to purchase automated external defibrillators (AEDs) and accessories;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI,
FLORIDA:
Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by
reference and incorporated as if fully set forth in this Section.
Section 2. The following new Special Revenue Fund is established and resources are appropriated
as described below:

APPROPRIATIONS:
$203,000
Section 3, This appropriation is contingent upon final grant approval by the State of Florida
Department of Health, Bureau of EMS and the City Manager's acceptance.
Section 4. Required matching funds, in the amount of $50,750, are allocated from the City's
Operating Budget Account Code No. 001000.921002.6.270.
Section 5. The City Manager is authorized{1} to execute the necessary documents, to implement
acceptance of the grant.
Section 6. This Resolution shalt become effective immediately upon its adoption and signature of
the Mayor.{2}

Footnotes:
{1} The herein authorization is further subject to compliance with all requirements that may be imposed
by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code
provisions.
{2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days
from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.
