(name of contracting company, organization, or indnidual) September 1 2004 WITNESSETH THAT: The CONTRACTOR shall begin performance of this agreement on , and shall complete (are) performance to the satisfaction of the COLLEGE no later than November 30, 2004 The CONTRACTOR certifies that he/she (are) has read and understands the agreements and statements on both sides of this form. The COLLEGE and the CONTRACTOR do mutual- ly agree that the CONTRACTOR shall furnish the following service to the COLLEGE, and that payment shall be made in accordance with the conditions of this agreement: DESCRIPTION OF SERVICE & TRAVEL ARRANGEMENTS (if applicable): For arrangements with consultants, actors, and other persons where a daily rate is to be paid, indicate: daily rate N/A x number of days N/A Total 24,000.00 City of Miami Police Department agrees tD provide Prepare Florida Part 1 and Part 2 training to its law enforcement personnel in individual 6 hour blocks (4 hour Part 2 PPE) and (2 hours Part 1 ICS) or in 4 hour blocks (4 hour Part 2 PPE) to completed by No> etttLet 30, 2004. The Contractor will provide class rosters to the School of Justice for grant reimbursement purposes. Class rosters consist of class date and time, printed names and signatures of participants Justice within 2 weeks of completion of each training class, with the last roster being received by the College no later than December 1, 2004. Contractor agrees to staff classes with a minimum of 20 students per class with an instructor/student ratio of 1 to 10. Certificates of completion for Prepare Florida will be issued by the School of Justice after receipt of class rosters. SCHE E AND PROCEDURE FOR PAYMENT - CONTRACTOR shall submit one invoice for the total amount due at the conclusion of the contract period. CONTRACTOR shall submit invoices periodically, relative to the amount of services provided, throughout the contract period. """ , REVIEW REVERSE SIDE FOR DETAILS The CONTRACTOR agrees, in fulfill this contract, to abide by all provisions of Section 202 of Executive Order 11246, as amended by Executive Order 11375 relative to Equal Employment Opportunity for all persons without regard to race, creed, color, national origin, or sex; and the implementing rules and regulations pertaining thereto The CONTRACTOR or the authorized representative the CONTRACTOR, certifies or affirms that all travel expenses included in this contract are, or will be, true and correct in every material matter, and that these expenses were actually incurred are necessary travel expenses in the performance of this contract. If the CONTRACTOR has received an advance airline ticket from the COLLEGE, the CONTRACTOR agrees to utilize said ticket as issued or to pay any additional costs incurred as a result of CONTRACTOR initiated changes. The CONTRACTOR agrees to abide by all pertinent IRS regulations. The CONTRACTOR shall hold harmless and indemnity the COLLEGE, their officers , agents and employees from any daims, demands, or causes of action except those causes of action arising out of or based solely upon the negligent acts, or omissions, or errors of the COLLEGE, their officers, agents , and employees. to extent and within the limit of 768.28 Florida Statute, as maybe amended. The CONTRACTOR, at its own cost, expense and risk, shall defend any legal proceedings that may be brought against the COLLEGE, their officers, agents and employees on any claim or demand against which any of them is held harmless and indemnified hereunder To extent and within the limit of 768.28 Florida Statute, as my be amended.
