We are also requested to submit a Certification Form. Enclosed you will find an executed Certification Form, as well as a copy of a letter dated September 23, 2004, from Mr. Michael L. Alston, Director of the Office for Civil Rights, advising us that our recently submitted Equal Employment Opportunity Plan (EEOP) had been reviewed and approved by the Office for Civil Rights.
