HomeMy WebLinkAboutR-04-0675City of Miami Legislation Resolution: R-04-0675 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 04-01031 Final Action Date: 10/14/2004 A RESOLUTION OF THE MIAMI CITY COMMISSION ACCEPTING THE RECOMMENDATION OF THE CITY MANAGER APPROVING THE FINDINGS OF THE EVALUATION COMMITTEE, PURSUANT TO REQUEST FOR PROPOSALS NO. 03-04-078, THAT THE FIRMS MOST QUALIFIED TO PROVIDE EMPLOYEE BENEFIT LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT PLANS, ARE, IN RANK ORDER: (1) MINNESOTA MUTUAL; (2) CIGNA; AND (3) METROPOLITAN LIFE; AUTHORIZING THE CITY MANAGER TO NEGOTIATE AN AGREEMENT WITH THE TOP -RANKED FIRM; FURTHER AUTHORIZING THE CITY MANAGER TO NEGOTIATE AN AGREEMENT WITH THE SECOND -RANKED FIRM, IN THE EVENT NEGOTIATIONS FAIL WITH THE TOP -RANKED FIRM, OR UNTIL AN AGREEMENT HAS BEEN SUCCESSFULLY NEGOTIATED; FURTHER DIRECTING THE CITY MANAGER TO PRESENT THE NEGOTIATED AGREEMENT TO THE CITY COMMISSION FOR CONSIDERATION. WHEREAS, pursuant to public notice, Request For Proposal ("RFP") No. 03-04-078 was issued to provide Employee Benefit Life and Accidental Death and Dismemberment ("AD&D") Plans, on a contract basis for three years, with the option to renew for two additional two-year periods, for the Department of Risk Management; and WHEREAS, the RFP for Employee Benefit Life and AD&D Plans provided for the current coverage levels, as well as, optional voluntary life products; and WHEREAS, RFPs were received on August 20, 2004, and the proposals were evaluated by an Evaluation Committee appointed by the City Manager which recommended that the proposal received from the following firms be placed in rank order: (1) Minnesota Mutual, (2) Cigna, and (3) Metropolitan Life and be accepted as the firms that the City will begin to negotiate with for these services; and WHEREAS, the parameters of the contract will be brought back before the City Commission for final approval once the terms have been successfully negotiated; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. The recommendation of the City Manager approving the findings of the Evaluation Committee, pursuant to RFP No. 03-04-078, that the firms most qualified to provide Employee Benefit Life and AD&D Plans, are, in rank order: (1) Minnesota Mutual; (2) Cigna; and (3) Metropolitan Life, is accepted. Section 3. The City Manager is authorized{1} to negotiate an Agreement with the top -ranked firm. The City Manager is further authorized{2} to negotiate an Agreement with the second -ranked firm, in the event negotiations fail with the top -ranked firm, or until an Agreement has been successfully negotiated. Section 5. The City Manager is directed to present the negotiated Agreement to the City Commission for consideration. Section 6. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{3} Footnotes: {1} The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions. {2} The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions. {3} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
