As Chairperson of the Evaluation Committee ("Committee") for the above services for the City of Miami, it is
my responsibility to offer the findings and recommendation of the Committee.
The City issued an RFP on August 20, 2004 for Employee Benefit Life & AD&D Plan requesting quotes on the
current coverage levels, as well as, optional voluntary life products. The cost of the voluntary program would
be paid for 100% by the employees. Responses for this RFP were due to the City Clerk by September 20, 2004.
The appointed Committee met and discussed the merits of the responsive proposals, and evaluated each
according to the plans proposed, pursuant to the City's RFP. Following discussion and deliberation, the
Committee, by consensus, recommends the following Proposer(s), in rank order:
1. Minnesota Mutual
2, Cigna
3. Metropolitan Life
We are requesting permission to negotiate with the number 1 firm. Should contract negotiations fail with the
top ranked firm, the Committee recommends the City negotiate with the second ranked firm. If negotiations
fail with the second ranked firm, then permission to negotiate with the third ranked firm. Thecontract will be
negotiated until it is found to be acceptable by the City. Once the contract is negotiated, it will be brought back
before commission for final approval.
