HomeMy WebLinkAboutR-04-0673City of Miami Legislation Resolution: R-04-0673 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 04-01084 Final Action Date: 10/14/2004 A RESOLUTION OF THE MIAMI CITY COMMISSION ACCEPTING THE RECOMMENDATION OF THE CITY MANAGER APPROVING THE FINDINGS OF THE EVALUATION COMMITTEE, PURSUANT TO REQUEST FOR PROPOSALS NO. 03-04-066, THAT THE FIRMS MOST QUALIFIED TO PROVIDE EMPLOYEE BENEFIT HEALTH PLANS, ARE CIGNA, FOR PART A, C, AND D SERVICES, AND UNITED BENEFITS/ COMPANION LIFE, FOR PART B SERVICES; AUTHORIZING THE CITY MANAGER TO EXECUTE AGREEMENTS, IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, WITH EACH FIRM FOR AN INITIAL THREE-YEAR PERIOD WITH THE OPTION TO RENEW FOR TWO ADDITIONAL TWO-YEAR PERIODS; ALLOCATING FUNDS FROM ACCOUNT CODE NO. 514401.623301.6.657. WHEREAS, pursuant to public notice, Request For Proposals ("RFP") No. 03-04-066 was issued to provide Employee Benefit Health Plans, on a contract basis for a three-year period, with the option to renew for two additional two-year periods, for the Department of Risk Management; and WHEREAS, the RFP for Employee Benefit Health Plans was divided into four (4) parts: Part A: Third Party Administrative Services, Part B: Stop Loss, Part C: Prescription Drug Services, and Part D: Employee Assistance Program Services; and WHEREAS, RFPs were received on May 17, 2004, and the proposals were evaluated by an Evaluation Committee appointed by the City Manager; and WHEREAS, contracts have been successfully negotiated with Cigna and United Benefits / Companion Life; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. The recommendation of the City Manager approving the findings of the Evaluation Committee, pursuant to RFP No. 03-04-066, that the firms most qualified to provide Employee Benefit Health Plans, are Cigna, for Part A, C, and D Services, and United Benefits/ Companion Life, for Part B Services, is accepted. Section 3. The City Manager is authorized{1} to execute Agreements, in a form acceptable to the City Attorney, with each firm for an initial three-year period with the option to renew for two additional two-year periods, with funds allocated from Account Code No. 514401.623301.6.657. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{2} Footnotes: {1} The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions. {2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
