Located along the Northwesterly side of SW 2 Avenue and Easterly of I-95
The City of Miami Plat and Street Committee, at its meeting of Apr111, 2004 approved the
above tentative plat subject to the following revisions being made to the tentative plat,
---------additional• information being provided and/or variances being granted. Pleasebeadvised
that the processing of your tentative plat cannot proceed until these conditions have
been satisfied.
1. In the location sketch:
a) Correct the zoning information, in accordance with the City of Miami Official
Zoning Atlas dated January 1,1999.
b) Reference Plat Book 1, Page 87 as PLAT OF BLOCK 42, SOUTH MIAMI.
c) Delete the statements: Refer to HOLLMAN PARK, PB 8-23 assessment roll.
2. A plat approval letter from the F.D.O.T. Is required at the time of final plat submittal.
A copy of the final plat must be submitted to the F.D.O.T., not a copy of the tentative
plat.
3. Delete the underlying subdivisions for the North -South Expressway.
4. An opinion of title for each adjacent remnant portion of unplatted land is required.
5. Be advised that current backup documentation will be required for all who execute
the final plat. A resolution for authority to execute documents and a Certificate of
Good Standing from the Florida Secretary of State is required, if applicable.
6. The notary acknowledgment portion of the application for tentative plat approval
must be completed. The notary acknowledgment portion and signature portion of
the application do not correspond to each other.
7. Does Brlckwell Towngate, LLC have a company seal? Provide a seal for Brickeli
Towngate, LLC, if applicable.

In addition to the above requirements, you should be aware of the following:
1. State and local laws require the installation of various physical improvements in the
public rights -of -way when property Is platted. These subdivision improvements
include paving, drainage, landscaping, sidewalks, etc. In some cases this could
represent a substantial Investment. on your part.
2. The alteration, relocation or installation of utilities such as storm and sanitary sewers,
electric, telephone, water, etc. caused by this plat will be at the property owners
expense. Also, utility easements may be required on the property being platted. •
3. A building permit will not be Issued on the property being platted until the final plat
is recorded. Also, the Certificate of occupancy for any building construction will be
issued only after all the required subdivision Improvements have been completed.
4. Approval for fire flow requirements must be obtained from the Fire -Rescue
Department prior tothe issuance.of.a building permit:-
5. Additional Items must be provided to the City of Miami Department of Public Works
before the final plat is submitted to the City Commission for approval. You will be
notified in writing as to what these Items are after the amount of the bond has been
determined for the necessary subdivision improvements.
6. Tentative plat approval Is only valid for one (1) year from the date of the Plat and
Street Committee meeting at which, time It was approved.

The accompanying Plat entitled CATHERINE'S PLACE was prepared by South Atlantic
Services of Florida. It is in correct form for submission to the City Commission and is forwarded
with a recommendation that it be approved.
PERTINENT INFORMATION REGARDING THE PLAT:
(1) The property platted is a subdivision of the property described in the attached Legal
Description. , The area platted consists of two (2) lots containing approximately 16,581
square feet (0.38t of an acre). It is zoned O.
(2) The location of the streets and their widths conform to the standards of the Department
of Public Works of the City of Miami, Florida.
(3) As certified to by Narciso Jose Ramirez, Registered Surveyor and Mapper, this Plat
complies with the plat filing laws of the State of Florida.
(4) The Certificate of Title Examination dated 1(/?/ f2' Off .41e , signed by

title to the property platted is correctly vested in Brickell Towngate, L.L.C., a Florida
limited liability company, and that the Plat has been correctly executed.

(6) In accordance with the requirements of Chapters 54 and 55 of the Code of the City of
Miami, Florida, a Letter of Credit, #SB20040030, in the amount of $20,752.00 has been
executed by Brickell Towngate, L.L.C., Principal, and Ocean Bank, This Letter of Credit
