The City of Miami Plat and Street Committee, at its meeting of March 4, 2004, approved
the above tentative plat subject to the following revisions being made to the tentative
plat, additional information being provided and/or variances being granted. Please be
advised that the processing of your tentative plat cannot proceed until these conditions
have been satisfied.
1. Provide a tie (distances and bearings) to a sectional comer.
2. Provide bearings and distances along the City monument line and along the centerline
for NW 14 Street.
3. Show the underlying legal description in the main sketch.
4. Add species (botanical) name to tree description.
5. Provide the Miami -Dade County Flood Criteria information; Include the plat book and
page numbers.
6. Describe the specific site development intent on the tentative plat.
7. Be advised that the final plat will have to make the following code requirement
statements under the Owner's Plat Restrictions:
a) The private road will be maintained by the subdivider and/or abutting property
owners.
b) No attempt shall be made to dedicate the private right-of-way to the public.
B. The private road will also have to be dedicated as an emergency access easement and
as a public utility easement. Verify with the Fire -Rescue Department that turn -around
has sufficient maneuvering area.
9. Be advised that current backup documentation will be required for all who execute
the final plat. A resolution for authority to execute documents and a Certificate of
Good Standing from the Secretary of State Is required, If applicable.
10. An opinion of title in the City of Miami opinion of title form must be provided at the
time of the final plat submittal. The opinion must cover from the beginning to a most
date.

11. The notary acknowledgment must be completed on the tentative plat application.
12. Existing concrete driveway must be modified to conform to the new_private road and
the number of parking spaces required at time of permitting of original duplex.
In addition to the above requirements, you should be aware of the following:
1. State and local laws require the installation of various physical improvements in the
public rights -of -way when property is platted. These subdivision improvements
include paving, drainage, landscaping, sidewalks, etc. In some cases this could
represent a substantial investment on your part.
2. The alteration, relocation or installation of utilities such as storm and sanitary sewers,
electric, telephone, water, etc. caused by this plat will be at the property owners
expense. Also, utility easements may be required on the property being platted.
3. A building permit will not be issued on the property being platted until the final plat
Is recorded. Also, the. Certificate of Occupancy for any. building .construction will be
issued only after all the required subdivision improvements have been completed.
4. Approval for fire flow requirements must be obtained from the Fire -Rescue
Department prior to the issuance of a building permit.
5. Additional items must be provided to the City of Miami Department of Public Works
before the flnal plat is submitted to the City Commission for approval. You will be
notified in writing as to what these Items are after the amount of the bond has been
determined for the necessary subdivision improvements.
6. Tentative plat approval is only valid for one (1) year from the date of the Plat and
street Committee meeting at which time it was approved.

sheet for the appropriate person to contact. '

LOCATED ON THE NORTH SIDE OF NW 14 STREET

A SUBDIVISION IN THE CITY OF MIAMI, FLORIDA
The accompanying Plat entitled KEW GARDENS FIRST ADDITION was prepared by
Nova Surveyors, Inc. It is in correct form for submission to the City Commission and is
forwarded with a recommendation that it be approved.
PERTINENT INFORMATION REGARDING THE PLAT:
(1) The property platted Is a replat of the property described in the attached Legal
Description. The area platted consists of two (2) lots and two (2) tracts containing
approximately 17,817 square feet (0.38± of an acre). It is zoned R-2.
(2) The location of the streets and their widths conform to the standards of the
Department of Public Works of the City of Miami, Florida.
(3) As certified to by John Ibarra, Registered Surveyor and Mapper, this Plat compiles
with the plat filing laws of the State of Florida.
(4) The Certificate of Title Examination dated �;e ,i3 ,loll , signed by
/— 2 6 .4/4.z. , Attorney, Indicates that the fee simple title
to the property platted is correctly vested In Pablo Cuello and Magdalia Cuello, his
wife, and that the Plat has been correctly executed.
(5) The area platted is not encumbered by a mortgage.

(6) In accordance with the requirements of Chapters 54 and 55, of the Code of the
City of Miami, Florida, a Cashier's Check In the amount of S19,501.00 has been
tendered by the owners, Pablo Cuello and Magdalia Cuelio, his wife. This Cashiers
Check has been deposited with the Director of Finance and will guarantee the
completion of the subdivision Improvements according to the provisions of the
Agreement between the City of Miami and the aforementioned 'owners. The
following Is required at the plat property:
